What is Head Circumference?

Head circumference refers to the largest measurement around the human skull, taken with a flexible, non-stretch tape measure across the forehead and over the most prominent point at the back of the head (occiput). It is a routine part of pediatric well-child visits and provides valuable information about brain and skull growth.

Why is Head Circumference Measured?

Head circumference serves as an indirect indicator of brain growth. During the first years of life, the brain grows rapidly, which is reflected in a corresponding increase in head circumference. Deviations from the expected growth pattern may signal developmental disorders or underlying medical conditions and should be evaluated by a healthcare professional.

Normal Values and Growth Charts

Head circumference is assessed using age- and sex-specific percentile charts. These charts show where an individual measurement falls relative to a reference population of the same age. Values between the 3rd and 97th percentile are generally considered normal. Equally important is the trend across multiple measurements over time, rather than any single reading.

  • Newborns: Average head circumference approximately 34–35 cm
  • At 6 months: approximately 43–44 cm
  • At 12 months: approximately 46–47 cm
  • At 2 years: approximately 48–49 cm
  • Adults: approximately 54–58 cm

Abnormal Findings

Microcephaly

Microcephaly is defined as a head circumference significantly below the age-appropriate norm (below the 3rd percentile or more than 2 standard deviations below the mean). It may indicate impaired brain development and can result from genetic factors, infections during pregnancy (e.g., Zika virus, rubella), or other causes.

Macrocephaly

Macrocephaly describes a head circumference significantly above the normal range (above the 97th percentile). Possible causes include hydrocephalus (accumulation of fluid in the brain), familial macrocephaly, or, in rare cases, metabolic disorders.

How is Head Circumference Measured?

The measurement is taken using a flexible, non-stretch tape measure placed just above the eyebrows at the front and around the largest part of the back of the head. For reliable documentation, the measurement should always be performed in the same manner by trained medical personnel.

Clinical Relevance in Adults

In adults, head circumference measurement has less diagnostic significance than in children, as skull growth is largely complete by around two years of age. However, it may be relevant for fitting protective helmets, in neurosurgical planning, or in the context of certain genetic syndromes.

When to See a Doctor?

Parents should seek medical advice if:

  • their child's head circumference falls significantly outside the normal range
  • head circumference grows unusually fast or shows no growth over time
  • additional symptoms appear, such as developmental delays, behavioral changes, or seizures
