What is GHRP-2?

GHRP-2 (Growth Hormone Releasing Peptide-2) is a synthetic six-amino-acid peptide (hexapeptide) that stimulates the secretion of growth hormone (GH, somatotropin) from the pituitary gland. It belongs to the class of compounds known as growth hormone secretagogues (GHS) and acts by binding to the ghrelin receptor (GHS-R1a). Developed in the 1980s, GHRP-2 is among the most extensively studied peptides of its class.

Mechanism of Action

GHRP-2 exerts its effects primarily through two mechanisms:

  • Binding to the GHS-R1a receptor in the hypothalamus and pituitary gland, triggering the release of growth hormone.
  • Inhibition of somatostatin, a natural inhibitor of GH release, which amplifies the GH pulse.

Compared to GHRP-6, GHRP-2 produces a stronger and more consistent stimulation of GH secretion. In addition, GHRP-2 can elevate IGF-1 levels (Insulin-like Growth Factor 1), which mediates many of the anabolic and regenerative effects associated with growth hormone.

Medical Applications and Research Context

GHRP-2 is primarily used in scientific research and is not approved as a licensed medication for general clinical use in most countries. The following areas of application are under investigation:

  • Diagnosis of growth hormone deficiency: GHRP-2 is used as a stimulation test to assess the pituitary gland capacity to release GH.
  • Cachexia and muscle atrophy: Research is exploring its use in conditions characterized by muscle wasting, such as chronic illness or aging.
  • Wound healing and tissue regeneration: Preclinical studies suggest potential regenerative properties.
  • Appetite stimulation: Similar to ghrelin, GHRP-2 can increase appetite, which may be beneficial in malnourished patients.

Pharmacological Properties

GHRP-2 is biologically active when administered subcutaneously or intravenously. Its half-life in circulation is short (approximately 15 to 60 minutes), which is why research protocols often involve multiple daily injections. Oral bioavailability is poor, as peptides are degraded in the gastrointestinal tract.

Side Effects and Risks

Since GHRP-2 is not an approved drug and is used primarily in research settings, its safety profile has not been fully established. Known and potential side effects include:

  • Elevated cortisol levels and prolactin release due to pituitary stimulation
  • Water retention (edema) caused by GH-mediated effects
  • Increased appetite through ghrelin-like activity
  • Tingling or numbness in the extremities
  • Fatigue following injection
  • Potential insulin resistance with chronic high-dose use

Uncontrolled use outside of clinical trials carries significant risks, as long-term human data are lacking.

Regulatory Status and Misuse Potential

GHRP-2 is listed by the World Anti-Doping Agency (WADA) as a prohibited substance due to its potential for misuse as a performance-enhancing agent in sport. In many countries, it is available exclusively for research purposes and is not approved for human use. Consumers should be aware that products sold as GHRP-2 may vary considerably in purity and dosage.
