What is D-Chiro-Inositol?
D-Chiro-Inositol (DCI) is a naturally occurring cyclic sugar compound belonging to the inositol family. It is an isomer of myo-inositol and plays a key role in the insulin signaling pathway in the human body. D-Chiro-Inositol is found in small amounts in various foods, including buckwheat, legumes, and citrus fruits.
Mechanism of Action
D-Chiro-Inositol acts as an inositolphosphoglycan mediator within the insulin signaling pathway. In the body, it is synthesized from myo-inositol by the enzyme epimerase. In individuals with insulin resistance, this conversion process is often impaired, leading to a relative DCI deficiency. DCI supports the activation of enzymes such as pyruvate dehydrogenase, which is involved in glucose metabolism, thereby promoting cellular glucose uptake and utilization.
Medical Applications
Polycystic Ovary Syndrome (PCOS)
The most well-known application of D-Chiro-Inositol is in the management of polycystic ovary syndrome (PCOS). Women with PCOS often exhibit elevated insulin resistance and a deficiency in DCI. Clinical studies demonstrate that supplementation with DCI – frequently combined with myo-inositol in a 40:1 ratio (myo-inositol to DCI) – can improve insulin sensitivity, reduce testosterone levels, and regulate the menstrual cycle.
Insulin Resistance and Type 2 Diabetes
In individuals with insulin resistance or an elevated risk of type 2 diabetes, D-Chiro-Inositol may contribute to improved glycemic control. Research suggests that DCI supplementation can lower fasting blood glucose levels and reduce HbA1c values.
Reproductive Health and Fertility
D-Chiro-Inositol is also used to support ovarian function and fertility in women with PCOS. Evidence suggests that an optimized inositol balance can improve oocyte quality and increase ovulation rates.
Dosage and Administration
D-Chiro-Inositol is typically used at doses of 300 mg to 1,200 mg per day, often in combination with myo-inositol. The most commonly studied combination follows a 40:1 ratio of myo-inositol to D-Chiro-Inositol, which mirrors the physiological ratio found in blood plasma. DCI is generally taken orally in capsule or powder form. It is advisable to consult a healthcare professional before starting supplementation.
Dietary Sources
In the diet, D-Chiro-Inositol is found primarily in the following foods:
- Buckwheat (particularly rich in DCI)
- Legumes (lentils, beans, chickpeas)
- Citrus fruits (oranges, grapefruits)

Side Effects and Safety
D-Chiro-Inositol is generally considered well tolerated at therapeutic doses. In rare cases, the following side effects may occur:
- Gastrointestinal discomfort (bloating, nausea, diarrhea)
- Headaches
- Dizziness at very high doses
Paradoxically, excessive DCI intake may negatively affect ovarian function, as an excess of DCI within oocytes can impair egg quality. Therefore, adherence to recommended dosages and the physiological ratio with myo-inositol is essential.
Interactions
When taken alongside blood glucose-lowering medications (e.g., metformin, insulin), the dosage should be monitored by a healthcare professional, as D-Chiro-Inositol may potentiate the glucose-lowering effect of these agents.
