What is Iron Pyrophosphate?
Iron pyrophosphate (also known as ferric pyrophosphate or iron(III) pyrophosphate) is an inorganic iron compound composed of iron and pyrophosphoric acid. It is primarily used as an iron source in food fortification and dietary supplements and is considered a well-tolerated alternative to other iron forms such as iron sulfate or iron gluconate.
Biological Functions of Iron
Iron is an essential trace element that fulfils numerous vital functions in the human body:
- Oxygen transport: Iron is a key component of hemoglobin in red blood cells and myoglobin in muscles, enabling the transport and storage of oxygen.
- Energy metabolism: Iron is involved in the activity of enzymes required for cellular energy production.
- Immune system: Adequate iron intake supports the normal function of the immune system.
- Cognitive function: Iron plays a role in brain development and function, especially in children.
Mechanism of Action and Bioavailability
Iron pyrophosphate exists in its trivalent form (Fe³³). In the gastrointestinal tract, it must first be reduced to divalent iron (Fe²²) before it can be absorbed via specific transport proteins in the intestinal lining. The bioavailability of iron pyrophosphate is somewhat lower than that of iron sulfate, but it is significantly better tolerated and causes fewer gastrointestinal side effects.
In micronized and liposomally encapsulated form, iron pyrophosphate demonstrates improved bioavailability. Liposomal iron pyrophosphate is taken up directly by intestinal cells, bypassing the classical absorption pathway and further enhancing tolerability.
Medical Applications and Uses
Treatment and Prevention of Iron Deficiency
Iron pyrophosphate is commonly used for the prevention and treatment of iron deficiency and iron deficiency anemia. Groups at particular risk who may benefit from supplementation include:
- Pregnant and breastfeeding women
- Infants and toddlers
- Women of childbearing age with heavy menstruation
- Vegetarians and vegans
- Endurance athletes
- Persons with chronic inflammatory bowel diseases
Food Fortification
Iron pyrophosphate is an approved food additive and is used for the fortification of foods such as table salt, flour, rice products, and infant foods. It offers the advantage of minimally affecting the taste and color of fortified products.
Dialysis Patients
A specialized medical application involves the use of iron pyrophosphate in dialysate solutions for patients with kidney failure who require regular hemodialysis. In this context, iron can be delivered directly into the bloodstream.
Recommended Daily Intake
The recommended daily iron intake varies by age, sex, and physiological status. According to the World Health Organization (WHO) and other health authorities, general reference values include:
- Adult men and postmenopausal women: approx. 10 mg/day
- Women of childbearing age: approx. 15 mg/day
- Pregnant women: approx. 27 mg/day
- Breastfeeding women: approx. 20 mg/day
- Children and adolescents: 8–15 mg/day depending on age

Tolerability and Side Effects
Iron pyrophosphate is considered particularly well-tolerated compared to other iron compounds. Possible, though rare, side effects at higher doses may include:
- Mild gastrointestinal discomfort (constipation, bloating)
- Dark coloration of stools (harmless)
- Nausea at very high doses
Compared to iron sulfate, these complaints occur significantly less frequently, making iron pyrophosphate especially suitable for sensitive individuals.
Interactions
The following interactions should be considered when taking iron pyrophosphate:
- Vitamin C enhances iron absorption and is frequently used in combination.
- Calcium, magnesium, and zinc can inhibit iron absorption – it is recommended to take them at different times.
- Phytic acid (found in whole grains and legumes) and tannins (in tea and coffee) can reduce iron uptake.
- Certain medications such as antacids, tetracyclines, or proton pump inhibitors can interfere with iron absorption.
