What is an Ecchymosis?
An ecchymosis (from Greek ekchymosis: outpouring of blood) is a flat area of discoloration on the skin or mucous membranes caused by blood leaking out of damaged blood vessels beneath the surface. In everyday language, it is commonly referred to as a bruise or contusion. Medically, ecchymoses are distinguished from petechiae (pinpoint bleedings) and purpura (small bleedings) by their larger size – typically more than one centimeter in diameter. As the body reabsorbs the leaked blood, the color changes progressively from blue-purple to green and finally to yellow-brown.
Causes
Ecchymoses can result from a wide variety of causes:
- Traumatic causes: Blunt force trauma such as bumps, falls, or crushing injuries are the most common causes. Small blood vessels (capillaries) or larger vessels rupture beneath an intact skin surface.
- Medication-related causes: Blood-thinning medications such as anticoagulants (e.g., warfarin, heparin) or antiplatelet agents (e.g., aspirin) significantly increase the tendency to bruise.
- Bleeding disorders: Conditions such as hemophilia, von Willebrand disease, or a low platelet count (thrombocytopenia) predispose individuals to ecchymoses.
- Age-related changes: In older adults, the skin becomes thinner and blood vessels more fragile, leading to so-called senile ecchymoses, which occur with minimal trauma.
- Underlying diseases: Liver disease, vitamin K deficiency, scurvy (vitamin C deficiency), or vasculitis (inflammation of blood vessels) can also cause ecchymoses.
- Medical procedures: Injections, blood draws, or surgical interventions may lead to localized ecchymoses.
Symptoms and Appearance
Ecchymoses appear as non-blanching skin discolorations – meaning the color does not fade when pressure is applied, which helps distinguish them from redness caused by dilated blood vessels. Typical features include:
- Flat skin discoloration ranging from blue-purple to green to yellow-brown as healing progresses
- Diameter greater than one centimeter
- Possible local swelling or tenderness
- No elevated skin temperature (unless an inflammatory cause is present)
Ecchymoses that appear spontaneously, without a recognizable cause, or that are unusually large or frequent should always be evaluated by a healthcare professional.
Diagnosis
Ecchymosis is typically a clinical diagnosis, made by visual examination. If an underlying condition is suspected, further testing may be required:
- Complete blood count (CBC): To assess platelet levels
- Coagulation studies: Including prothrombin time (PT/INR) and activated partial thromboplastin time (aPTT)
- Liver function tests: If liver disease is suspected
- Vitamin K and vitamin C levels: If nutritional deficiencies are a concern
- Imaging: Ultrasound or MRI may be used to evaluate deeper hematomas
Treatment
Treatment of ecchymosis depends on its underlying cause:
Treatment for Traumatic Ecchymosis
- Cold application: Applying an ice pack (wrapped in a cloth to protect the skin) immediately after injury helps limit bleeding and reduce swelling.
- Elevation: Raising the affected limb can support faster healing.
- Topical preparations: Heparin-based creams or arnica preparations may be applied locally to support reabsorption of the bruise.
Treatment for Underlying Medical Conditions
- Bleeding disorders require targeted therapy of the primary condition (e.g., clotting factor replacement in hemophilia).
- Medication-induced ecchymoses may require dose adjustment or a change in medication, under medical supervision.
- Nutritional deficiencies (vitamin K, vitamin C) are addressed through targeted supplementation and dietary changes.
When to See a Doctor
Medical evaluation is recommended if:
- Ecchymoses appear without a clear cause or injury
- Bruising is unusually large, frequent, or worsening
- Additional symptoms are present such as fever, joint pain, or unusual fatigue
- You are taking blood-thinning medications and experience unexpected bleeding
- The ecchymosis does not resolve within three weeks or continues to enlarge
