What Is Skin Turgor?
Skin turgor refers to the natural elasticity and firmness of the skin, which is determined by the fluid content within the tissue. Healthy skin turgor means the skin is adequately hydrated and returns quickly to its original shape after being briefly stretched or pinched. Reduced skin turgor is an important clinical warning sign for dehydration (fluid deficiency) or other underlying medical conditions.
How Is Skin Turgor Tested?
Testing skin turgor is a simple physical examination technique that requires no special equipment. A clinician pinches a small fold of skin – typically on the back of the hand, the forearm, or the abdomen – lifts it briefly, and then releases it.
- Normal turgor: The skin fold smooths out immediately (within 1–2 seconds).
- Reduced turgor: The skin fold remains elevated and returns slowly to its original position, suggesting fluid deficiency.
- Increased turgor: The skin feels unusually firm or swollen, which may indicate edema (fluid accumulation in the tissue).
Causes of Reduced Skin Turgor
Reduced skin turgor occurs when there is insufficient fluid in the body tissues. Common causes include:
- Dehydration due to inadequate fluid intake
- Severe vomiting or diarrhea leading to significant fluid loss
- High fever with excessive sweating
- Diabetes mellitus with increased urinary output
- Kidney disease
- Malnutrition and significant weight loss
- Natural aging, as older skin retains less moisture
Limitations of the Skin Turgor Test
It is important to note that the skin turgor test has certain limitations. In older adults, skin turgor is naturally reduced even without dehydration, due to the age-related decline in collagen and hyaluronic acid within the skin. Obesity can also make the assessment more difficult. For this reason, skin turgor is always evaluated alongside other clinical signs, such as dry mouth, reduced urine output, or low blood pressure.
Clinical Significance
Skin turgor is a well-established component of the clinical examination and is widely used to quickly assess a patient's hydration status. It plays an especially important role in emergency medicine, pediatrics, and geriatrics. When skin turgor is significantly reduced, further diagnostic steps may be required, such as blood tests (e.g., electrolytes, hematocrit) or measurement of urine specific gravity.
Treatment of Reduced Skin Turgor
Treatment depends on the underlying cause:
- Mild dehydration: increased fluid intake through water or electrolyte-rich drinks
- Severe dehydration: intravenous fluid administration in a clinical setting
- Underlying conditions (e.g., diabetes): targeted medical treatment of the root cause
- Skin care to support the skin barrier, particularly in elderly individuals
