During the menopause, a woman's hormonal balance changes significantly. The decline in the female sex hormone estrogen has a major impact on bone metabolism. Estrogen plays a key role in protecting bones by inhibiting bone breakdown. When estrogen levels fall, the balance between bone-resorbing cells (osteoclasts) and bone-forming cells (osteoblasts) is disrupted – bone breakdown begins to dominate. This leads to a loss of bone density and increases the long-term risk of osteoporosis and fractures.
The primary trigger for bone loss during menopause is the drop in estrogen levels. Additional contributing factors include:
Bone loss is typically symptom-free in its early stages. Only when bone density has decreased significantly do the following signs tend to appear:
The most important diagnostic tool for detecting bone loss is bone densitometry, commonly performed using DXA scanning (Dual-Energy X-ray Absorptiometry). This measures bone mineral density at the lumbar spine and hip, expressed as a T-score:
Additional tests may include blood tests (e.g., calcium, vitamin D, bone turnover markers) and imaging such as X-rays.
A calcium-rich diet is essential for bone health. A daily intake of approximately 1,000–1,200 mg of calcium is recommended (sources: dairy products, leafy green vegetables, almonds, calcium-rich mineral water). Adequate vitamin D levels are equally important, as vitamin D promotes calcium absorption in the gut. Health authorities generally recommend 800 IU of vitamin D per day for adults.
Weight-bearing exercises and strength training (e.g., hiking, jogging, dancing) stimulate bone formation and help maintain bone density. Regular physical activity also improves muscle strength and balance, reducing the risk of falls.
For women at high risk of osteoporosis or those already diagnosed, several medication options are available:
