Bone health during menopause refers to the condition and changes in bone strength that occur during and after the hormonal transition phase in a woman's life. As estrogen – the primary female sex hormone – declines, women lose bone density much more rapidly than at any other point in their lives. This significantly increases the risk of osteoporosis (bone loss) and related fractures.
Bone is a living tissue that is constantly being remodeled. Estrogen plays a central role in this process by inhibiting bone breakdown (by cells called osteoclasts) and promoting bone formation (by osteoblasts). When estrogen levels fall during menopause, this balance is disrupted and bone breakdown prevails.
Bone loss often progresses for years without noticeable symptoms. Only when bone density has dropped significantly do complications become apparent:
Bone density measurement (osteodensitometry / DXA scan) is the standard method for diagnosing osteoporosis. It measures bone mineral density at the lumbar spine and hip and compares the result to a reference value for a healthy young adult woman (the T-score).
Blood tests (e.g., calcium, vitamin D, parathyroid hormone) can also provide information about bone metabolism.
A calcium-rich diet forms the foundation for strong bones. For women aged 50 and over, a daily calcium intake of 1,000–1,200 mg is recommended. Good sources include dairy products, green leafy vegetables, sesame seeds, and calcium-rich mineral water. Vitamin D is essential for calcium absorption in the gut; supplementation is recommended when a deficiency is confirmed.
Strength training and weight-bearing activities (e.g., hiking, dancing, stair climbing) stimulate bone formation and improve muscle strength, reducing the risk of falls. Regular physical activity is one of the most effective strategies for maintaining bone density.
When fracture risk is elevated or osteoporosis is established, several medications are available:
