The death cap mushroom (Amanita phalloides) is the most dangerous poisonous fungus in Europe and is responsible for the majority of fatal mushroom poisonings worldwide. It belongs to the genus Amanita and grows widely in deciduous and mixed forests. Despite its extreme toxicity, it is frequently confused by non-experts with edible mushrooms such as the common button mushroom or the blusher.
The death cap mushroom typically has the following characteristics:
The greenish coloration can vary and is not a reliable identifying feature. The volva (the sac-like sheath at the base of the stem) is the most important distinguishing characteristic from harmless mushrooms.
The death cap mushroom contains several highly toxic substances:
As little as 0.1 mg of amanitin per kilogram of body weight (equivalent to approximately 20–30 g of fresh mushroom) can be lethal for an adult. A single mushroom contains enough toxin to kill a human being.
Poisoning by the death cap mushroom typically progresses through four phases:
After ingestion, no symptoms appear initially. This deceptive symptom-free period often leads those affected to delay seeking medical attention.
Severe gastrointestinal symptoms develop: intense nausea, vomiting, colicky abdominal pain, and cholera-like diarrhea with massive fluid loss.
Gastrointestinal symptoms appear to subside. However, serious damage to the liver and kidneys is already occurring internally, detectable through laboratory tests.
Severe liver failure (jaundice, bleeding tendency, hepatic encephalopathy) and kidney failure develop. Without timely treatment or liver transplantation, the poisoning can be fatal.
Diagnosis is based on:
If poisoning by the death cap mushroom is suspected, emergency services or a poison control center must be contacted immediately. Every suspected case is a medical emergency.
There is no specific antidote for amatoxins. Treatment includes:
The death cap mushroom is frequently confused with:
Important: When in doubt, never eat a mushroom. Cooking, pickling, or drying does not render the death cap mushroom safe. The toxins are heat-stable.
