Lupus nephritis is an inflammation of the kidneys caused by the autoimmune disease systemic lupus erythematosus (SLE). In SLE, the immune system mistakenly attacks the body's own tissues. When this attack targets the kidneys, it leads to lupus nephritis -- a progressive kidney disorder that can result in permanent damage or kidney failure. Approximately 40 to 60 percent of all SLE patients develop lupus nephritis at some point during their illness.
Lupus nephritis is caused by the deposition of immune complexes -- aggregates of antibodies and antigens produced in excess during SLE -- within the kidney's filtering units, known as glomeruli. These deposits trigger an inflammatory response that damages the delicate filtration tissue of the kidneys.
The symptoms of lupus nephritis often develop gradually and may go unnoticed for some time. Common signs include:
Diagnosing lupus nephritis requires a combination of clinical evaluation, laboratory testing, and kidney biopsy.
Lupus nephritis is classified according to the internationally recognized ISN/RPS classification into six classes:
Treatment of lupus nephritis aims to suppress the overactive immune response, preserve kidney function, and prevent flares. Therapy is tailored to the severity and class of the disease.
During the acute phase, intensive treatment is initiated to rapidly control inflammation:
Once the acute flare is controlled, long-term therapy is initiated to prevent relapse:
In cases of severe kidney damage, dialysis or, in end-stage disease, a kidney transplant may become necessary.
The course of lupus nephritis varies greatly between individuals. With modern treatment, many patients achieve complete or partial remission. However, a proportion of patients remain at risk of developing chronic kidney failure. Regular monitoring of kidney function, blood pressure, and laboratory parameters is therefore essential. Early diagnosis and consistent treatment significantly improve long-term outcomes.
