What is a Neural Tube Defect?
A neural tube defect (NTD) is a congenital malformation of the central nervous system that occurs when the neural tube – the embryonic precursor to the brain and spinal cord – fails to close completely during early pregnancy. This closure normally occurs between days 22 and 28 after fertilisation, often before a woman even knows she is pregnant.
Neural tube defects are among the most common serious birth defects worldwide. The clinical presentation varies widely depending on the location and extent of the incomplete closure.
Types of Neural Tube Defects
- Spina bifida: The most common form. The vertebral canal in the spinal region does not close completely. Subtypes range from the mild spina bifida occulta to the more severe myelomeningocele, in which spinal cord tissue protrudes through the skin.
- Anencephaly: Large parts of the brain and skull are absent. This condition is incompatible with life.
- Encephalocele: Brain tissue protrudes through a gap in the skull bones.
Causes and Risk Factors
The exact causes are not yet fully understood. NTDs result from a multifactorial interaction of genetic and environmental factors:
- Folic acid deficiency: Insufficient intake of folic acid (vitamin B9) in early pregnancy is the most important modifiable risk factor.
- Genetic factors: Family history and specific gene variants (e.g., in the MTHFR gene) increase the risk.
- Medications: Certain antiepileptic drugs such as valproic acid can raise the risk.
- Maternal diabetes mellitus, especially if poorly controlled.
- Obesity in the mother.
- Elevated body temperature (fever) during the first trimester.
Symptoms and Consequences
Symptoms depend greatly on the type and severity of the defect:
- Paralysis and sensory disturbances of the legs (in spina bifida)
- Bladder and bowel dysfunction
- Hydrocephalus (accumulation of fluid in the brain)
- Learning difficulties and cognitive impairments
- Orthopaedic problems such as clubfoot or hip dislocation
Diagnosis
Many neural tube defects can now be detected during pregnancy:
- Ultrasound (sonography): Routine prenatal scans can reveal structural abnormalities from the second trimester onwards.
- Alpha-fetoprotein (AFP) in maternal blood: Elevated levels may indicate an open neural tube defect.
- Amniocentesis: Analysis of amniotic fluid to confirm findings.
- MRI (magnetic resonance imaging): Provides detailed images of the developing nervous system.
Treatment
While there is no cure, early treatment can significantly improve quality of life:
- Surgical closure: In open spina bifida, the defect is closed surgically after birth or, in specialised centres, prenatally (before birth).
- Shunt implantation: A shunt (drainage valve) is inserted to manage hydrocephalus.
- Physiotherapy and rehabilitation: To promote mobility and independence.
- Urological monitoring: Regular assessment of bladder function.
- Interdisciplinary care: Neurology, paediatrics, orthopaedics, psychology, and social work collaborate closely.
Prevention
The most effective preventive measure is taking folic acid before and during pregnancy:
- The WHO and major national health authorities recommend taking at least 400 µg of folic acid daily, starting at least 4 weeks before conception and continuing through the first trimester.
- Women at higher risk (e.g., previous pregnancy affected by an NTD, use of antiepileptic drugs) may be advised to take 4 mg daily.
- Folic acid-rich foods include: green leafy vegetables, legumes, whole grains, and liver.
