The perineum is an anatomical region located at the base of the pelvis. It extends from the external genitalia (the vaginal opening in women or the scrotum in men) to the anus. While the structure differs between males and females, the perineum serves essential functions in both sexes, including pelvic floor support and control of bladder and bowel function.
The perineum consists of skin, connective tissue, muscles, and nerves. It forms the inferior boundary of the pelvis and is functionally closely linked to the pelvic floor.
Anatomically, the perineum is divided into two main regions:
Key muscles of the perineum include the external anal sphincter, which controls bowel continence, and various pelvic floor muscles that support the bladder, bowel, and uterus (in women).
The perineum serves several important physiological functions:
During childbirth, the perineum may sustain tears, classified into four degrees based on depth and severity. A perineal tear of the first degree involves only the skin, while a fourth-degree tear extends through the anal sphincter and rectal mucosa. An episiotomy is a surgical incision made to facilitate delivery and prevent uncontrolled tearing.
A perineal abscess is a localized collection of pus in the perineal area caused by bacterial infection. It presents with severe pain, swelling, redness, and warmth. Surgical drainage is usually required.
Pelvic floor dysfunction can result from childbirth, chronic straining, or aging. Common symptoms include urinary incontinence, pelvic organ prolapse, and discomfort in the perineal area.
Perineal fistulas are abnormal channels connecting internal organs or structures to the skin surface in the perineal region. They often develop as a complication of infection, inflammatory bowel disease, or surgery.
Perineal conditions are typically assessed using:
