What is the Z-Score in bone density measurement?
The Z-Score is a statistical value used in bone density measurement (also called bone densitometry or DXA scanning). It indicates how many standard deviations a person's bone density differs from the average of an age- and sex-matched reference group. Unlike the T-Score, which compares bone density to the peak bone mass of young adults, the Z-Score accounts for natural age-related bone changes. It is therefore the preferred measurement in children, adolescents, premenopausal women, and men under the age of 50.
Calculation and Interpretation
The Z-Score is calculated as follows:
- Z-Score = (measured bone density − mean of age group) / standard deviation of age group
The following reference values are used for interpretation:
- Z-Score ≥ −2.0: Bone density is within the expected range for the patient's age
- Z-Score < −2.0: Bone density is below the expected range for the patient's age – may indicate a secondary cause of bone loss
A negative Z-Score means bone density is lower than average for the age group, while a positive value indicates above-average bone density.
Clinical Applications
The Z-Score is particularly relevant in the following clinical situations:
- In children and adolescents, where bone development is still ongoing
- In premenopausal women, where hormone-related bone loss is not yet the primary concern
- In men under 50 years of age
- To evaluate secondary osteoporosis, meaning bone loss caused by underlying conditions such as Crohn's disease, celiac disease, long-term corticosteroid use, or hormonal disorders
Difference Between Z-Score and T-Score
Both values are obtained during bone density measurement but serve different clinical purposes:
- T-Score: Compares bone density to the peak bone mass of young healthy adults. Used to diagnose osteopenia and osteoporosis in postmenopausal women and older men.
- Z-Score: Compares bone density to peers of the same age. Used to identify bone loss exceeding the expected age-related decline and to investigate secondary causes.
Causes of a Low Z-Score
A Z-Score below −2.0 may point to various underlying conditions or risk factors, including:
- Chronic inflammatory diseases (e.g., rheumatoid arthritis, Crohn's disease)
- Long-term use of corticosteroids
- Hormonal disorders (e.g., hypogonadism, hyperthyroidism, hyperparathyroidism)
- Malabsorption syndromes (e.g., celiac disease, lactose intolerance with calcium deficiency)
- Physical inactivity or immobility
- Eating disorders (e.g., anorexia nervosa)
- Calcium or vitamin D deficiency
Diagnosis and Next Steps
Bone density measurement using DXA (Dual-Energy X-ray Absorptiometry) is the gold standard for determining the Z-Score. Measurements are typically taken at the lumbar spine and the femoral neck. When the Z-Score is abnormal, current guidelines recommend further diagnostic workup to identify potential secondary causes, including:
- Blood and urine tests (calcium, phosphate, vitamin D, parathyroid hormone, thyroid function)
- Bone turnover markers (e.g., crosslinks, osteocalcin)
- Genetic testing if a hereditary bone disorder is suspected
Treatment Approaches for a Low Z-Score
Treatment is directed at the underlying cause and may include:
- Treating the underlying condition (e.g., correcting thyroid dysfunction, managing malabsorption)
- Optimising calcium and vitamin D intake through diet and supplementation
- Regular physical activity, especially weight-bearing and resistance exercises to support bone health
- Adjusting bone-damaging medications (e.g., reducing or replacing corticosteroids)
- In selected cases: pharmacological therapy (e.g., bisphosphonates, denosumab) as determined by a physician
