Things worth knowing about "Agony"

Agony refers to the final phase of life in which the body's vital functions progressively fail. It is a natural part of the dying process, recognized in palliative care.

What Is Agony?

The term agony derives from the Greek word agonia, meaning struggle or death struggle. In modern medicine, agony describes the final stage of the dying process, during which the body's vital organ systems fail one after another. This phase may last minutes, hours, or in rare cases a few days.

Agony is not a sudden event but a gradual, biologically governed process characterized by specific physical changes. It occurs in severe, incurable illnesses, after life-threatening injuries, or in old age as a natural conclusion of life.

Causes and Onset of the Agonal Phase

The agonal phase is triggered when the body can no longer maintain its basic functions. Common causes and triggers include:

  • Advanced cancer with multi-organ failure
  • Severe heart disease such as terminal heart failure
  • Respiratory failure in end-stage chronic lung disease
  • Severe infections such as end-stage sepsis
  • Organ failure (liver, kidneys, brain)
  • Advanced old age with natural cessation of vital functions
  • Severe trauma or brain injury

Signs and Symptoms of Agony

The agonal phase is recognizable by a number of typical physical changes that are important for family members and caregivers to know:

Changes in Breathing

  • Cheyne-Stokes respiration: irregular breathing with pauses (apnoeic episodes)
  • Death rattle caused by secretion pooling in the airways
  • Shallow, very slow breaths

Circulation and Skin

  • Pale, greyish, or yellowish skin tone
  • Bluish discoloration of the lips, fingers, and toes (cyanosis)
  • Cold, mottled skin, especially on the extremities
  • Barely measurable blood pressure and weak pulse

Consciousness and Nervous System

  • Progressive loss of consciousness or deep clouding of awareness
  • Little or no response to verbal stimulation
  • Glassy, half-open eyes
  • Loss of bladder and bowel control

Additional Signs

  • Marked decrease in urine output, progressing to complete anuria
  • Cooling of the body
  • Inability to swallow or speak

Diagnosis and Clinical Assessment

The recognition of the agonal phase is a clinical assessment made by experienced physicians and nursing staff. There is no single diagnostic test; rather, the totality of clinical signs is evaluated. In palliative and hospice care, structured observational tools such as evolved versions of the Liverpool Care Pathway or the AMBER Care Bundle are used to identify the dying phase and initiate appropriate care.

Care and Treatment During Agony

The goal of medical care during the agonal phase is not cure but comfort and dignity. Treatment focuses on:

Symptom Control

  • Pain relief: opioid analgesics (e.g., morphine) to relieve pain and breathlessness
  • Anxiety relief: benzodiazepines (e.g., midazolam) for agitation or distress
  • Secretion reduction: anticholinergics (e.g., hyoscine butylbromide) to reduce the death rattle
  • Discontinuation of non-essential medications and diagnostic procedures

Nursing and Human Companionship

  • Regular mouth care to relieve dry mouth
  • Repositioning to prevent pressure sores
  • Calm, familiar surroundings with subdued lighting
  • Inclusion and support of family members and loved ones
  • Chaplaincy and psychosocial support as desired

Communication with Family Members

Family members should be informed that hearing may be preserved until the very end. Words of comfort, gentle touch, and the opportunity to say goodbye can be meaningful for the dying person.

Agony in Palliative Medicine

Palliative medicine has made the care of dying people a central specialty. Its aim is to relieve suffering, preserve human dignity, and support both the dying person and their family throughout the agonal phase. Hospices and specialist palliative care units provide particularly intensive and personal care during this time.
