Things worth knowing about "Breast refusal"

Breast refusal describes a baby rejecting the mother's breast during breastfeeding. Causes are varied and in most cases treatable.

What is Breast Refusal?

Breast refusal occurs when a baby refuses to latch onto or feed from the mother's breast. This can happen suddenly or develop gradually and can affect infants of all ages. It is not a disease in itself, but rather a behavioral response that often signals an underlying cause. For breastfeeding mothers, this experience can be emotionally distressing, as it may interfere with both nutrition and the bonding process between mother and child.

Causes

The reasons for breast refusal are wide-ranging. Possible causes related to the baby include:

  • Pain during sucking, for example due to teething, ear infections, oral thrush (a fungal infection), or a blocked nose
  • Change in the taste of breast milk, caused by hormonal changes in the mother, medications, new foods, or a new pregnancy
  • Nipple confusion, when the baby is introduced to bottles or pacifiers in addition to breastfeeding
  • Overactive letdown (excess milk flow), which can overwhelm the baby
  • Stress responses to changes in the environment or daily routine
  • Growth spurts and shifting feeding behavior

Possible causes on the mother's side include:

  • Changed body scent (e.g., due to new cosmetics or perspiration)
  • Alterations in milk composition due to hormonal changes
  • Stress or emotional strain

Signs and Symptoms

Common signs of breast refusal include:

  • The baby turns away from the breast
  • The baby cries or screams when offered the breast
  • The baby latches briefly and then pulls off
  • The baby refuses to open their mouth or bites the nipple
  • The baby appears restless or tense during breastfeeding attempts

Important note: If the baby is generally calm, drinks enough from a bottle, and is gaining weight appropriately, the refusal may simply be a temporary phase.

Diagnosis

Medical evaluation is not always necessary, but is recommended if:

  • the baby is losing weight or not gaining weight adequately
  • the baby shows signs of pain or illness
  • the refusal persists for more than a few days

In such cases, a pediatrician should be consulted to rule out organic causes such as ear infections, oral thrush, or gastrointestinal problems. An International Board Certified Lactation Consultant (IBCLC) can also provide valuable professional support.

Treatment and Measures

Management of breast refusal depends on the underlying cause:

  • Create a calm feeding environment: A familiar, low-stimulation setting can encourage the baby to feed.
  • Skin-to-skin contact (bonding): Frequent skin-to-skin contact can help reawaken the baby's interest in breastfeeding.
  • Try different breastfeeding positions: A change in hold -- such as the cradle hold or rugby hold -- may help.
  • Feeding during drowsiness: Some babies latch more easily when they are tired or half asleep.
  • Treat the underlying cause: If an organic issue such as an ear infection or thrush is identified, it should be treated directly.
  • Pumping breast milk: To maintain milk supply, mothers can express milk and offer it by spoon, cup, or bottle.

In most cases, breast refusal is a temporary phase. With patience and professional guidance, breastfeeding can often be successfully resumed.

When to Seek Medical Advice

Parents should seek medical attention promptly if:

  • the baby is losing significant weight
  • the baby appears lethargic or excessively irritable
  • the baby shows signs of infection (fever, rash)
  • breast refusal is accompanied by intense or prolonged crying
