What is the CAGE Test?
The CAGE Test is a short, standardized screening tool designed to quickly identify problematic alcohol use or alcohol dependence. The acronym CAGE stands for the key words in its four questions: Cut down, Annoyed, Guilty, and Eye-opener. Developed in 1968 by John Ewing, it is one of the most widely used screening instruments in addiction medicine and general practice worldwide.
The Four CAGE Questions
Patients are asked the following four questions:
- C (Cut down): Have you ever felt you should cut down on your drinking?
- A (Annoyed): Have you ever been annoyed by people criticizing your drinking?
- G (Guilty): Have you ever felt guilty or bad about your drinking?
- E (Eye-opener): Have you ever had a drink first thing in the morning to steady your nerves or get the day started?
Scoring and Interpretation
Each Yes answer scores one point. The total score ranges from 0 to 4:
- 0 points: No indication of problematic alcohol use.
- 1 point: Possible indication; further assessment is recommended.
- 2 or more points: Clinically significant indication of alcohol misuse or dependence; a more thorough diagnostic evaluation is strongly recommended.
A threshold score of 2 or more is widely regarded in clinical literature as a positive screening result, indicating potentially harmful alcohol use.
Clinical Applications
The CAGE Test is commonly used in the following medical settings:
- General practice and outpatient clinics
- Psychiatric and psychosomatic facilities
- Emergency departments and inpatient admissions
- Addiction counseling centers
Due to its brevity and simplicity, the CAGE Test is highly practical for everyday clinical use. However, it does not replace a comprehensive diagnostic assessment. A positive result should be followed up with in-depth interviews, structured tools such as the AUDIT (Alcohol Use Disorders Identification Test), or laboratory investigations.
Strengths and Limitations of the CAGE Test
Strengths
- Very brief and quick to administer (under 1 minute)
- No additional equipment or materials required
- High sensitivity for alcohol dependence in clinical populations
- Simple language, easy to understand for patients
Limitations
- Lower sensitivity for mild or moderate alcohol misuse
- Not designed to quantify the amount of alcohol consumed
- May yield false-negative results if patients minimize their drinking
- Less validated for certain population groups (e.g., adolescents, older adults)
