Catalpol is an iridoid glycoside – a secondary plant metabolite found in various medicinal plants. It is most abundantly present in the root of Rehmannia glutinosa (Chinese foxglove), a plant used in Traditional Chinese Medicine (TCM) for centuries. Catalpol has also been identified in plant genera such as Plantago, Veronica, and Catalpa.
Chemically, catalpol is a bicyclic monoterpene glycoside with the molecular formula C15H22O10. It is one of the most extensively researched bioactive plant compounds and is being actively investigated for its therapeutic potential.

Catalpol has been studied in multiple research projects for its antidiabetic properties. It can stimulate insulin secretion from pancreatic beta cells, improve peripheral glucose uptake, and reduce inflammation in adipose tissue, which plays a role in type 2 diabetes.
Preclinical data suggest that catalpol may have beneficial effects on osteoporosis and inflammatory joint diseases such as arthritis, by favoring the balance between bone formation and bone resorption.

In Traditional Chinese Medicine, the catalpol-containing root of Rehmannia glutinosa (Chinese: Dihuang) has been used for more than 2,000 years. Traditionally, it is applied to tonify the kidney and liver, purify the blood, and treat symptoms such as dizziness, tinnitus, night sweats, and general weakness.
There is currently no officially standardized dosage recommendation for catalpol as an isolated compound in humans, as most studies have been conducted in animal or cell models. In traditional preparations based on Rehmannia glutinosa, typically 9–30 g of dried root per day are used, which corresponds to a substantial amount of catalpol.
Catalpol-containing plant extracts are generally considered well tolerated at customary doses. Adverse effects at excessively high doses may include gastrointestinal complaints. Interactions with blood glucose-lowering medications are theoretically possible and should be monitored. Pregnant and breastfeeding women, as well as individuals with pre-existing conditions, should consult a physician before use.
Catalpol is currently in a predominantly preclinical research phase. Results from cell and animal studies are promising; however, further clinical trials in humans are needed to confirm efficacy, optimal dosage, and long-term safety. Catalpol is considered an interesting candidate for the development of new herbal therapeutics, particularly in the areas of neuroprotection and metabolic diseases.
