What are cholagogues?
A cholagogue (also spelled cholagog) is a substance that promotes the secretion or flow of bile from the liver and gallbladder into the small intestine. The term derives from the Greek words chole (bile) and agogos (leading or driving). In medicine, two main types of bile-stimulating agents are distinguished:
- Choleretics: Substances that increase bile production by the liver cells (hepatocytes).
- Cholagoguese (cholekinetics): Substances that stimulate the contraction of the gallbladder and promote bile flow into the duodenum.
The role of bile in digestion
Bile is a digestive fluid produced by the liver and stored in the gallbladder. It is released into the small intestine in response to dietary fat intake. Bile emulsifies dietary fats, making them accessible to fat-digesting enzymes called lipases. In addition, bile facilitates the absorption of fat-soluble vitamins (A, D, E, and K) and plays a key role in excreting metabolic waste products such as bilirubin.
Medical uses
Cholagogue substances are used for:
- Digestive complaints such as bloating, feeling of fullness, and impaired fat digestion
- Mild gallbladder and bile duct disorders
- Supporting liver function under increased metabolic load
- Stimulating digestive activity after surgical procedures
Mechanism of action
Cholagogues act through different mechanisms depending on their type:
- Choleretic effect: Stimulation of hepatocytes to produce more bile, often triggered by bitter compounds or secretory signals.
- Cholekinetic effect: Promotion of gallbladder contraction via the release of cholecystokinin (CCK), a gut hormone triggered by dietary fats and proteins.
Examples of cholagogue substances
Plant-based substances
- Artichoke extract (Cynara scolymus): Increases bile production and has liver-protective properties.
- Milk thistle (Silybum marianum): Acts as a choleretic and hepatoprotective agent.
- Turmeric (Curcuma longa): Curcumin stimulates gallbladder activity and supports fat digestion.
- Dandelion (Taraxacum officinale): Promotes bile secretion and flow.
- Ginger (Zingiber officinale): Supports digestion with mild choleretic effects.
Synthetic and semi-synthetic substances
- Ursodeoxycholic acid (UDCA): A bile acid that improves bile flow and is used in certain liver conditions such as primary biliary cholangitis.
- Dehydrocholic acid: A synthetic choleretic used to stimulate bile secretion.
Contraindications and safety considerations
Cholagogue substances are not appropriate in all situations. Important contraindications include:
- Gallstones (cholelithiasis): Stimulating gallbladder contractions may trigger a gallstone attack.
- Bile duct obstruction (cholestasis): Use is dangerous when bile outflow is blocked.
- Acute gallbladder inflammation (cholecystitis): Cholagogues should not be used during acute episodes.
- Severe liver disease: Use may be contraindicated depending on the severity of the condition.
Before taking any cholagogue preparation, patients should consult a physician or pharmacist, especially if they have existing liver or gallbladder conditions.
