Things worth knowing about "Fat-free mass"

Fat-free mass includes all body components without fat tissue, such as muscles, bones, and organs. It is a key indicator of overall health and physical performance.

What is fat-free mass?

Fat-free mass (FFM), also referred to as lean body mass (LBM), describes the total weight of all body tissues that contain no fat. This includes skeletal muscle, bones, organs, skin, blood, and body fluids. It is a central measurement in body composition analysis and is considered alongside body fat mass, which represents the proportion of stored fatty tissue in the total body weight.

Importance for health

Fat-free mass is a key indicator of physical health and performance. A higher fat-free mass, particularly a greater proportion of muscle tissue, is associated with a lower risk of metabolic diseases, cardiovascular disease, and osteoporosis. As people age, fat-free mass naturally declines – a process known as sarcopenia – which can significantly affect quality of life and independence.

Components of fat-free mass

  • Skeletal muscle: the largest component; essential for movement, strength, and metabolism
  • Bone: provides structural support and stores minerals such as calcium
  • Organs: including the liver, heart, kidneys, and lungs
  • Skin and connective tissue: protective and structural functions
  • Blood and body fluids: transport of nutrients and oxygen throughout the body

Measurement of fat-free mass

Several methods are used to assess fat-free mass:

  • Bioelectrical impedance analysis (BIA): A widely used, non-invasive method that measures the electrical conductivity of body tissues to estimate fat and muscle mass.
  • DEXA scan (dual-energy X-ray absorptiometry): A highly accurate imaging technique that precisely distinguishes between bone, fat, and muscle tissue.
  • Hydrostatic weighing: A classic method that calculates body volume by measuring weight underwater.
  • Skinfold thickness measurement: A simple technique using calipers to estimate subcutaneous fat and infer fat-free mass.

Factors influencing fat-free mass

Several factors affect an individual's fat-free mass:

  • Age: From the age of 30 onwards, muscle mass typically decreases by approximately 3–8% per decade without sufficient physical activity.
  • Sex: Men generally have higher muscle mass than women, largely due to higher testosterone levels.
  • Nutrition: Adequate protein intake is essential for maintaining and building muscle mass.
  • Physical activity: Resistance training and regular exercise promote the preservation and growth of fat-free mass.
  • Hormonal factors: Hormones such as testosterone, growth hormone, and insulin play important roles in muscle and bone development.
  • Health conditions: Chronic illness, malnutrition, or prolonged immobility can significantly reduce fat-free mass.

Fat-free mass and nutrition

A protein-rich diet supports the maintenance and development of fat-free mass. The World Health Organization (WHO) and major nutrition bodies recommend a protein intake of approximately 0.8 g per kilogram of body weight per day for healthy adults. Older adults and athletes may require higher amounts. In addition to protein, micronutrients such as vitamin D, calcium, and magnesium are important for bone and muscle health.

Clinical relevance

In clinical settings, fat-free mass is used to assess nutritional status, calculate medication dosages (e.g., in chemotherapy), and monitor disease progression in conditions such as cancer, heart failure, and kidney disease. A reduction in fat-free mass is recognized as a significant risk factor for increased mortality in critically ill patients.
