What Is the FIB-4 Index?

The FIB-4 Index (Fibrosis-4 Index) is a simple, non-invasive diagnostic score used to estimate the degree of liver fibrosis – the scarring of liver tissue caused by chronic inflammation or injury. The FIB-4 Index is particularly valuable because it requires no liver biopsy and is based entirely on routine blood test values.

Originally developed for patients with chronic hepatitis C, the score is now widely used across various liver conditions, including non-alcoholic fatty liver disease (NAFLD/MASLD) and alcoholic liver disease.

How Is the FIB-4 Index Calculated?

The FIB-4 Index is calculated using a defined formula that incorporates the following parameters:

  • Age of the patient (in years)
  • AST (Aspartate Aminotransferase) – a liver enzyme (in U/L)
  • Platelet count (in 10&sup9;/L)
  • ALT (Alanine Aminotransferase) – another liver enzyme (in U/L)

The formula is:

FIB-4 = (Age × AST) / (Platelets × √ALT)

All required values are part of a standard blood panel and are routinely measured in clinical practice.

Interpreting the Results

The FIB-4 Index is interpreted using established cut-off values, which may vary slightly between guidelines:

  • FIB-4 < 1.30: Low risk for advanced fibrosis (F0–F1). Significant liver scarring is unlikely.
  • FIB-4 1.30 – 2.67: Indeterminate zone – further testing is recommended (e.g., elastography or liver biopsy).
  • FIB-4 > 2.67: High risk for advanced fibrosis or cirrhosis (F3–F4). Urgent further evaluation is indicated.

In older patients (aged 65 and above), adjusted cut-off values should be applied, as age directly influences the score and may otherwise overestimate the degree of fibrosis.

Clinical Applications

The FIB-4 Index is used across various clinical settings:

  • Chronic hepatitis B and C: First-line screening tool for fibrosis staging
  • NAFLD/MASLD and NASH/MASH: Risk stratification in fatty liver disease
  • Alcoholic liver disease: Monitoring of disease progression and treatment response
  • General and internal medicine: Cost-effective screening in everyday clinical practice

Advantages and Limitations of the FIB-4 Index

Advantages

  • Non-invasive: no biopsy required
  • Cost-effective: based on standard laboratory values
  • Rapid availability: no specialized equipment needed
  • Well-validated across diverse patient populations
  • Recommended in international clinical guidelines (e.g., EASL, AASLD)

Limitations

  • Results may be influenced by other conditions (e.g., muscle disease, hemolysis)
  • Indeterminate results require additional diagnostic workup
  • Age-related adjustments are necessary in elderly patients
  • Cannot replace liver biopsy in ambiguous cases

FIB-4 Compared to Other Fibrosis Scores

Several other non-invasive scores are available for liver fibrosis assessment:

  • APRI Score (AST-to-Platelet Ratio Index): simpler but generally less accurate than FIB-4
  • NFS (NAFLD Fibrosis Score): specifically developed for NAFLD, incorporates additional parameters such as BMI and blood glucose
  • ELF Test (Enhanced Liver Fibrosis Test): serum marker-based test, more expensive but with higher precision
  • Liver biopsy: gold standard for fibrosis diagnosis, but invasive

Studies consistently show that the FIB-4 Index is one of the most reliable and cost-effective methods for initial fibrosis screening.
