Things worth knowing about "Bone loss during menopause"

Bone loss during menopause is a common condition in which declining estrogen levels reduce bone density and increase the risk of osteoporosis.

What is bone loss during menopause?

During the menopause, a woman's hormonal balance changes significantly. The decline in the female sex hormone estrogen has a major impact on bone metabolism. Estrogen plays a key role in protecting bones by inhibiting bone breakdown. When estrogen levels fall, the balance between bone-resorbing cells (osteoclasts) and bone-forming cells (osteoblasts) is disrupted – bone breakdown begins to dominate. This leads to a loss of bone density and increases the long-term risk of osteoporosis and fractures.

Causes

The primary trigger for bone loss during menopause is the drop in estrogen levels. Additional contributing factors include:

  • Calcium and vitamin D deficiency
  • Physical inactivity and sedentary lifestyle
  • Smoking and excessive alcohol consumption
  • Genetic predisposition and family history
  • Low body weight (low body mass index)
  • Early menopause (before age 45)
  • Long-term use of corticosteroids

Symptoms

Bone loss is typically symptom-free in its early stages. Only when bone density has decreased significantly do the following signs tend to appear:

  • Back pain, often caused by vertebral fractures
  • Loss of height and increasing curvature of the spine (kyphosis)
  • Fractures from minimal trauma (e.g., wrist, hip, spine)
  • General heaviness or pain in the bones

Diagnosis

The most important diagnostic tool for detecting bone loss is bone densitometry, commonly performed using DXA scanning (Dual-Energy X-ray Absorptiometry). This measures bone mineral density at the lumbar spine and hip, expressed as a T-score:

  • T-score ≥ -1.0: Normal bone density
  • T-score between -1.0 and -2.5: Osteopenia (precursor to osteoporosis)
  • T-score ≤ -2.5: Osteoporosis

Additional tests may include blood tests (e.g., calcium, vitamin D, bone turnover markers) and imaging such as X-rays.

Treatment and prevention

Nutrition

A calcium-rich diet is essential for bone health. A daily intake of approximately 1,000–1,200 mg of calcium is recommended (sources: dairy products, leafy green vegetables, almonds, calcium-rich mineral water). Adequate vitamin D levels are equally important, as vitamin D promotes calcium absorption in the gut. Health authorities generally recommend 800 IU of vitamin D per day for adults.

Exercise and physical activity

Weight-bearing exercises and strength training (e.g., hiking, jogging, dancing) stimulate bone formation and help maintain bone density. Regular physical activity also improves muscle strength and balance, reducing the risk of falls.

Medical treatments

For women at high risk of osteoporosis or those already diagnosed, several medication options are available:

  • Bisphosphonates (e.g., alendronate, risedronate): inhibit bone-resorbing osteoclasts
  • Hormone replacement therapy (HRT): can compensate for estrogen decline and slow bone loss; suitability must be assessed individually
  • Denosumab: a monoclonal antibody that inhibits bone resorption
  • Raloxifene: a selective estrogen receptor modulator (SERM)
  • Teriparatide: actively stimulates bone formation

Lifestyle modifications

  • Quitting smoking
  • Limiting alcohol intake
  • Fall prevention measures at home (non-slip mats, appropriate footwear)
