Liver stiffness (also referred to as hepatic stiffness or liver elasticity) describes the degree of hardening of the liver tissue. A healthy liver is relatively soft and elastic. When the liver is damaged – for example by chronic inflammation, excessive alcohol consumption, or metabolic disorders – it forms scar tissue, a process known as fibrosis. This scar tissue increases the measurable stiffness of the organ. Measuring liver stiffness is an important diagnostic tool for assessing the severity of liver fibrosis or cirrhosis without the need for a tissue biopsy.

Liver stiffness is today primarily measured using non-invasive methods that can replace a liver biopsy in many cases.
Transient elastography, widely known by the brand name FibroScan, is the most commonly used method. An ultrasound probe sends a sound wave into the liver tissue. The speed at which this wave propagates is directly proportional to the hardness of the tissue. Stiffness is expressed in kilopascals (kPa). Normal values are approximately 2–7 kPa. Higher values indicate advanced fibrosis or cirrhosis.
Magnetic resonance elastography combines MRI technology with elastography principles. It provides a highly accurate, wide-area map of liver stiffness and is considered particularly precise, though it is more complex and costly than FibroScan.
ARFI elastography is an ultrasound-based method that can be integrated directly into modern ultrasound devices, allowing targeted measurement in a selected region of the liver.
In addition to imaging-based elastography, blood tests can provide indirect indications of the degree of fibrosis – for example, the FIB-4 score (based on age, liver enzyme levels, and platelet count) or the APRI score.
