What is the MCH Value?

The MCH value (Mean Corpuscular Hemoglobin) is a laboratory parameter measured as part of a complete blood count (CBC). It reflects the average amount of hemoglobin contained within a single red blood cell (erythrocyte), expressed in picograms (pg). Hemoglobin is the red pigment responsible for transporting oxygen throughout the body. The MCH value helps physicians identify and classify different types of anemia (low red blood cell function).

Normal Range of MCH

For adults, the MCH value is generally considered normal when it falls between 28 and 34 picograms (pg) per red blood cell. Slight variations may occur depending on the laboratory. Age-specific reference ranges apply to children.

Causes of an Elevated MCH Value

An elevated MCH value (above 34 pg) is referred to as hyperchromia, meaning the red blood cells contain more hemoglobin than normal. Common causes include:

  • Vitamin B12 deficiency: Leads to macrocytic anemia with enlarged, hemoglobin-rich red blood cells
  • Folate deficiency: Another frequent cause of macrocytic anemia
  • Chronic alcohol abuse: Negatively affects blood cell production
  • Liver disease: Can disrupt erythropoiesis (the formation of red blood cells)
  • Hypothyroidism: An underactive thyroid gland can cause changes in blood parameters
  • Certain medications: For example, methotrexate or other cytostatic drugs

Causes of a Low MCH Value

A low MCH value (below 28 pg) is called hypochromia, indicating that red blood cells contain less hemoglobin than normal. Common causes include:

  • Iron deficiency: The most common cause; without sufficient iron, hemoglobin cannot be produced
  • Iron utilization disorders: For example, in chronic inflammation or infectious diseases
  • Thalassemia: An inherited disorder in which hemoglobin production is impaired
  • Lead poisoning: Can inhibit heme synthesis
  • Sideroblastic anemia: A disorder of iron utilization in the precursor cells of red blood cells

Diagnosis and Clinical Relevance

The MCH value is determined as part of a complete blood count and is always interpreted alongside other parameters. The most relevant accompanying values include:

  • MCV (Mean Corpuscular Volume): Average red blood cell size
  • MCHC (Mean Corpuscular Hemoglobin Concentration): Average hemoglobin concentration within red blood cells
  • Hemoglobin (Hb): Total amount of hemoglobin in the blood
  • Hematocrit: Proportion of red blood cells in total blood volume

Only by considering all blood parameters together can a physician make a reliable diagnosis and classify the type of anemia (e.g., microcytic, normocytic, or macrocytic).

Treatment for Abnormal MCH Values

Treatment always depends on the underlying cause:

  • For iron deficiency anemia: Dietary adjustments and/or iron supplements taken orally or intravenously
  • For vitamin B12 deficiency: Supplementation via tablets, nasal spray, or injections
  • For folate deficiency: Folic acid supplementation
  • For underlying conditions: Treatment of the root cause (e.g., thyroid therapy, liver treatment)

Important: Abnormal MCH values should always be evaluated by a physician. Self-treatment without a proper diagnosis is not recommended.
