What is Moxa Therapy?

Moxa therapy, also known as moxibustion, is a thousands-of-years-old treatment method rooted in Traditional Chinese Medicine (TCM). It involves the targeted application of heat to specific acupuncture points or body regions. The heat source is dried and processed mugwort herb (Artemisia vulgaris or Artemisia argyi), which is shaped into so-called moxa sticks or moxa cones and slowly burned.

In TCM, it is believed that illness arises from an imbalance or blockage in the flow of life energy (Qi). The heat generated by moxibustion is intended to stimulate the Qi, harmonise the flow of energy, and thereby support the body's healing processes.

Forms of Moxa Therapy

Moxibustion can be applied in several ways, depending on the treatment goal and the condition of the patient:

  • Direct moxibustion: A small moxa cone is placed directly on the skin and ignited. This method is more intense and is less commonly used today.
  • Indirect moxibustion: A moxa stick is held several centimetres above the skin without direct contact. This is the most common and gentle form of application.
  • Moxa on the needle: A piece of moxa herb is placed on an already-inserted acupuncture needle and burned, allowing heat to penetrate deeply into the acupuncture point.
  • Moxa with ginger or salt: A slice of ginger or a layer of salt is placed between the skin and the moxa cone to regulate intensity and introduce additional therapeutic compounds.

Areas of Application

Moxa therapy is used in TCM and increasingly in integrative medicine for a wide range of complaints:

  • Musculoskeletal pain: Back pain, joint pain, osteoarthritis, and muscle tension
  • Digestive problems: Irritable bowel syndrome, diarrhoea, bloating, and chronic stomach issues
  • Gynaecological complaints: Menstrual discomfort, pelvic pain, and correction of breech presentation during pregnancy
  • General weakness: Fatigue, weakened immune defence, and chronic exhaustion
  • Nervous system conditions: Numbness and peripheral neuropathies

Mechanism of Action

From a Western medical perspective, it is discussed that the local heat generated by moxibustion triggers various physiological effects:

  • Promotion of blood circulation in the treated area
  • Relaxation of muscles and connective tissue
  • Activation of the immune system through local inflammatory stimuli
  • Stimulation of nerve endings and possible influence on the central nervous system

In addition, the combustion products of mugwort contain volatile substances such as artemisinin and various essential oils, which may exert effects through the skin or via inhalation.

Scientific Evidence

The body of scientific evidence for moxa therapy is growing but not yet conclusive. Clinical studies have shown positive results particularly in the following areas:

  • Breech presentation correction: Several studies and a Cochrane review suggest that moxibustion at acupuncture point BL67 (Zhiyin) may help to encourage a baby to turn from breech position.
  • Chronic pain: Some controlled trials demonstrate effectiveness for back pain and knee pain due to osteoarthritis.
  • Digestive disorders: Initial evidence of effectiveness for irritable bowel syndrome and chronic colitis.

Further research is needed to fully establish the efficacy and underlying mechanisms of moxibustion.

Application and Safety

Moxa therapy should be performed by a trained practitioner in TCM or integrative medicine. Important notes for patients include:

  • The treatment should take place in a well-ventilated room, as the burning of mugwort produces smoke.
  • In certain conditions such as acute inflammation, high fever, specific stages of pregnancy, or in patients with sensitive skin, the therapy should not be used or should be applied only with special caution.
  • Smokeless moxa products (e.g., heat lamps or infrared applications) are an alternative for patients with respiratory sensitivity.
