Things worth knowing about "Nail bed inflammation"

Nail bed inflammation is a painful infection of the tissue surrounding the nail, commonly affecting fingers or toes. It can be caused by bacteria or fungi and requires prompt treatment.

What Is Nail Bed Inflammation?

Nail bed inflammation (medically known as paronychia or panaritium) is an infection or inflammation of the soft tissue surrounding the nail, including the nail fold and nail bed. It can affect both fingernails and toenails, typically causing redness, swelling, and pain around the nail. In more severe cases, pus can accumulate, forming an abscess.

Causes

Nail bed inflammation can be classified as either acute or chronic, depending on the cause:

Acute Nail Bed Inflammation

  • Bacterial infections: The most common cause is infection with Staphylococcus aureus, which enters through small breaks in the skin.
  • Torn cuticles, nail biting, or broken nails
  • Improper manicure or pedicure techniques
  • Foreign bodies under or beside the nail

Chronic Nail Bed Inflammation

  • Fungal infections (especially Candida species), often seen in people whose hands are frequently exposed to water or chemicals
  • Skin conditions such as psoriasis or eczema
  • Prolonged moisture or irritant exposure
  • Weakened immune system (e.g., in diabetes or during immunosuppressive therapy)

Symptoms

Common signs and symptoms of nail bed inflammation include:

  • Redness and swelling of the nail fold
  • Tenderness or throbbing pain around the nail
  • A feeling of warmth in the affected area
  • Pus formation (abscess) in more severe cases
  • Nail discoloration or nail separation in chronic cases

Diagnosis

Nail bed inflammation is usually diagnosed clinically, meaning a doctor can identify it by visual examination and palpation. In some cases, a swab from the affected area may be taken to identify the specific pathogen (bacteria or fungi). If a deeper infection is suspected, imaging such as an X-ray may be recommended to rule out involvement of deeper structures.

Treatment

Home Remedies and Mild Cases

  • Warm water soaks several times a day can improve circulation and provide relief in mild cases.
  • Keeping the affected finger or toe clean and protected
  • Avoiding further pressure or irritation

Medication

  • Antibiotics: For bacterial infections, a doctor may prescribe topical or oral antibiotics (e.g., flucloxacillin or amoxicillin-clavulanate).
  • Antifungals: Fungal infections are treated with antifungal creams or nail lacquers (e.g., clotrimazole, ciclopirox).
  • Over-the-counter pain relievers such as ibuprofen or paracetamol can help manage discomfort.

Surgical Treatment

If an abscess has developed, a minor surgical procedure to drain the pus may be necessary. This is typically performed under local anaesthesia and provides immediate relief.

Prevention

  • Trim nails regularly and carefully, avoiding cutting them too short
  • Do not tear or cut the cuticles aggressively
  • Dry hands thoroughly after contact with water or chemicals
  • Wear protective gloves during cleaning or wet work
  • Avoid biting or picking at nails

When to See a Doctor

Medical attention should be sought if pain is severe, significant pus is present, fever develops, or the inflammation spreads to a larger area. People with diabetes or a compromised immune system should consult a doctor early, as complications can arise more quickly.
