What is Newborn Care?

Newborn care refers to all nursing, medical, and hygienic practices carried out during the first weeks of a newborn's life. According to the World Health Organization (WHO), the neonatal period covers the first 28 days of life. During this time, infants are particularly vulnerable and depend on attentive care from parents, midwives, and healthcare professionals.

The goals of newborn care are to protect the health of the infant, support healthy development, and guide parents in their new caregiving role.

Body Hygiene and Skin Care

A newborn's skin is delicate and requires special attention. Key aspects of physical care include:

  • Bathing: A full bath should only be given after the umbilical cord stump has fallen off. Until then, parents can gently clean the baby with a moist cloth. Water temperature should be 37–38 °C (98.6–100.4 °F).
  • Umbilical cord care: The cord stump must be kept clean and dry. It typically falls off within 7–14 days. Signs of infection such as redness, swelling, or foul-smelling discharge should be watched for.
  • Diaper area: Frequent diaper changes (after every bowel movement or urination) protect against diaper rash. Mild barrier creams may be used as needed.
  • Skin care: Newborns are naturally protected by vernix caseosa. Harsh soaps and fragranced products should be avoided.

Feeding the Newborn

Nutrition is a central element of newborn care. The WHO recommends exclusive breastfeeding for the first six months of life, where possible.

  • Breastfeeding: Breast milk provides all necessary nutrients, antibodies, and growth factors. Newborns should be fed 8–12 times per day in the first weeks of life.
  • Formula feeding: Commercially prepared infant formula is a suitable alternative when breastfeeding is not possible.
  • Vitamin D supplementation: Health authorities in many countries recommend daily vitamin D supplementation of 400–500 IU for all newborns to prevent rickets, regardless of feeding method.

Sleep and Safe Sleep Environment

Newborns sleep an average of 16–18 hours per day. A safe sleep environment is critical for the prevention of Sudden Infant Death Syndrome (SIDS):

  • The baby should always be placed on its back to sleep.
  • The sleep surface should be firm and flat – no soft bedding, pillows, or loose blankets in the cot.
  • Room temperature should be kept between 16 and 18 °C (61–64 °F).
  • A smoke-free environment is essential.

Medical Check-Ups and Screening

Several important examinations are performed immediately after birth and in the first days of life:

  • APGAR score: Assessment of the newborn at 1, 5, and 10 minutes after birth, evaluating Appearance, Pulse, Grimace, Activity, and Respiration.
  • Newborn metabolic screening: A blood test performed on day 2–3 of life screens for more than 20 congenital metabolic disorders, such as phenylketonuria and hypothyroidism.
  • Hearing screening: Hearing ability is assessed using otoacoustic emissions (OAE) before hospital discharge.
  • Routine check-ups (U1, U2): In many countries, standardized well-baby examinations are performed immediately after birth and again between days 3 and 10 of life.

Bonding and Emotional Care

In addition to physical care, emotional closeness is an indispensable part of newborn care. Skin-to-skin contact (kangaroo care) promotes bonding, stabilizes the infant's body temperature, and supports breastfeeding. Research shows that early skin-to-skin contact has positive effects on neurological development and immune function.

Warning Signs and When to Seek Medical Help

Parents should seek medical attention promptly if any of the following warning signs occur:

  • Fever above 38 °C (100.4 °F) or body temperature below 36 °C (96.8 °F)
  • Breathing difficulties or bluish discoloration of the lips
  • Severe vomiting or diarrhea
  • Jaundice (icterus) persisting beyond 14 days or rapidly worsening
  • Signs of umbilical cord infection
  • Feeding difficulties or inadequate weight gain
