Things worth knowing about "Estrogen deficiency and bones"

Estrogen deficiency weakens bones and significantly raises the risk of osteoporosis. Learn how hormonal loss affects bone density and what can be done to protect skeletal health.

Estrogen and Its Role in Bone Metabolism

Estrogen is a key female sex hormone that not only governs reproductive function but also plays a vital role in bone metabolism. It maintains the balance between bone-forming cells (osteoblasts) and bone-resorbing cells (osteoclasts). As long as estrogen levels are sufficient, bone density remains stable.

When estrogen levels drop – most commonly during menopause or due to other causes – this balance is disrupted. Bone resorption outpaces bone formation, resulting in a loss of bone mass and structural integrity.

Causes of Estrogen Deficiency

  • Menopause: The most common cause. As menstruation ceases, the ovaries dramatically reduce estrogen production.
  • Premature ovarian insufficiency: Affects some women before the age of 40.
  • Surgical removal of the ovaries (oophorectomy): Causes an abrupt and complete drop in estrogen levels.
  • Underweight and eating disorders: Body fat contributes to estrogen production; extreme weight loss can suppress hormone levels.
  • Intense athletic training: Can lead to hormonal imbalances and absence of menstruation (amenorrhea).
  • Certain medications: Including aromatase inhibitors used in breast cancer treatment and GnRH analogues.

Effects on Bone Health

Without adequate estrogen, osteoclasts are no longer sufficiently inhibited, leading to accelerated bone resorption. In the first five to seven years following menopause, bone density can decrease by up to 20 percent. This can result in:

  • Osteopenia: A reduction in bone density that precedes osteoporosis.
  • Osteoporosis: Significant loss of bone mass with a markedly increased fracture risk.
  • Fractures: Most commonly affecting the spine, hip, and wrist.

Symptoms

Bone loss is often asymptomatic in its early stages. As the condition progresses, the following symptoms may appear:

  • Back pain caused by vertebral compression fractures
  • Loss of height and stooped posture (kyphosis)
  • Fractures from minor falls or everyday activities
  • General bone pain and tenderness

Diagnosis

The primary diagnostic tool is bone density measurement (DXA scan), also called dual-energy X-ray absorptiometry. It measures bone mineral density at the lumbar spine and hip. Results are expressed as a T-score:

  • T-score above -1.0: Normal bone density
  • T-score between -1.0 and -2.5: Osteopenia
  • T-score below -2.5: Osteoporosis

Blood tests measuring hormone levels, calcium, vitamin D, and bone turnover markers, as well as imaging studies, may also be used.

Treatment and Prevention

Hormone Replacement Therapy (HRT)

Hormone replacement therapy is one of the most effective strategies to prevent estrogen-related bone loss. Estrogen inhibits excess osteoclast activity and helps preserve bone density. Treatment decisions should always be individualized and discussed with a healthcare provider, weighing personal benefits and risks.

Calcium and Vitamin D

Calcium is the most important structural mineral in bone. Adults should aim for 1,000 to 1,200 mg of calcium daily through dietary sources such as dairy products, leafy greens, and almonds. Vitamin D is essential for calcium absorption in the gut and for maintaining bone health. The WHO recommends ensuring adequate vitamin D intake, particularly for older adults and postmenopausal women.

Pharmacological Treatment

For established osteoporosis, several medications are available:

  • Bisphosphonates (e.g., alendronate, risedronate): Inhibit bone resorption by osteoclasts.
  • Denosumab: A monoclonal antibody that blocks osteoclast activity.
  • Raloxifene: A selective estrogen receptor modulator (SERM) that mimics the bone-protective effects of estrogen.
  • Teriparatide: A parathyroid hormone analogue that stimulates new bone formation.

Lifestyle and Physical Activity

Regular physical activity – especially resistance training and weight-bearing exercises such as walking, hiking, or dancing – stimulates bone formation and improves muscle strength, thereby reducing the risk of falls and fractures. Smoking cessation and limiting alcohol intake are also strongly recommended, as both accelerate bone loss.
