What is Osgood-Schlatter Syndrome?
Osgood-Schlatter syndrome (also called Osgood-Schlatter disease) is a painful inflammation of the tibial tuberosity – the bony prominence just below the kneecap where the patellar tendon attaches. It is one of the most common causes of knee pain in growing children and adolescents, particularly those who are physically active in sports.
Causes
During periods of rapid growth spurts in puberty, bones, tendons, and muscles are especially vulnerable to overuse. Repetitive pulling forces from activities such as jumping, running, and rapid direction changes cause the patellar tendon to repeatedly stress the still-developing growth plate at the top of the shinbone, leading to small tears and inflammation.
- Intensive sports participation (soccer, basketball, gymnastics, athletics)
- Rapid bone growth during adolescence
- Muscular imbalances, especially tightness of the quadriceps muscle
- Poor jumping or running technique
Symptoms
The hallmark symptom is a sharp or aching pain directly below the kneecap that worsens with physical activity and improves with rest. A visible and palpable bony bump may develop at the affected site over time.

- Tenderness directly below the kneecap
- Visible swelling or bony prominence at the tibial tuberosity
- Symptoms that ease with rest
- Can affect one or both knees
Diagnosis
Diagnosis is usually made clinically, based on the physical examination and the patient's history. Tenderness at the tibial tuberosity is the key finding. Imaging may be used to confirm the diagnosis or rule out other conditions.
- X-ray: May show ossification or fragmentation of the tibial tuberosity
- Ultrasound: Useful for visualizing tendon and soft tissue inflammation
- MRI (Magnetic Resonance Imaging): Provides detailed assessment of tendon, bone, and cartilage in unclear cases
Treatment
In most cases, Osgood-Schlatter syndrome resolves on its own once bone growth is complete. Treatment focuses on managing symptoms and avoiding overloading the affected area.
Conservative Measures
- Activity modification or rest: Temporary reduction or avoidance of pain-triggering activities
- Ice application: Applying ice after exercise to reduce inflammation and pain
- Physiotherapy: Stretching and strengthening exercises for the quadriceps and calf muscles
- Pain relief: Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen for significant pain (under medical supervision)
- Knee strap or patellar tendon band: To reduce stress at the tendon insertion point
Surgical Treatment
Surgery is only rarely required – typically in cases where a painful bony fragment beneath the tendon persists after growth has completed.
Prognosis
The overall prognosis of Osgood-Schlatter syndrome is very good. The vast majority of those affected become pain-free once skeletal growth is complete. A permanent, usually painless bony prominence at the tibial tuberosity may remain as a harmless outcome. Only a very small proportion of patients continue to experience symptoms into adulthood.
