Things worth knowing about "Osteoporosis Early Detection"

Osteoporosis early detection helps identify bone loss before fractures occur. Learn which screening methods are recommended and who is most at risk.

What Is Osteoporosis Early Detection?

Osteoporosis is a condition in which bone density decreases, making bones more fragile and prone to fractures. Early detection aims to identify abnormal bone loss at the earliest possible stage – ideally before any fracture occurs. Since osteoporosis often develops silently over many years without noticeable symptoms, targeted screening is especially important.

Why Is Early Detection So Important?

Osteoporosis is frequently diagnosed only after a fracture – such as a vertebral or hip fracture – has already occurred. These fractures can have serious consequences, including chronic pain, loss of mobility, and increased mortality. Early diagnosis enables timely interventions to prevent fractures and slow down bone loss.

Risk Factors for Osteoporosis

Certain groups are at higher risk for developing osteoporosis and should be screened early:

  • Women after menopause (postmenopausal)
  • Men and women aged 70 and older
  • Individuals with calcium or vitamin D deficiency
  • Long-term use of corticosteroids (cortisone)
  • Family history of osteoporosis or fractures
  • Low body weight or eating disorders
  • Smoking and excessive alcohol consumption
  • Underlying conditions such as rheumatoid arthritis, diabetes, or thyroid disorders

Methods of Osteoporosis Early Detection

Bone Density Measurement (DXA)

The most important diagnostic tool for early detection is Dual-Energy X-ray Absorptiometry (DXA or DEXA). This technique uses a low dose of X-ray radiation to measure bone mineral density at the lumbar spine and hip. The result is expressed as a T-score:

  • T-score ≥ -1.0: Normal bone density
  • T-score between -1.0 and -2.5: Osteopenia (reduced bone density, a precursor to osteoporosis)
  • T-score ≤ -2.5: Osteoporosis

Fracture Risk Assessment with FRAX

The FRAX tool (Fracture Risk Assessment Tool) developed by the WHO calculates an individual's 10-year risk of a major osteoporotic fracture based on risk factors such as age, sex, body mass index, and bone density. It helps clinicians determine whether treatment is necessary.

Laboratory Tests

Supplementary blood and urine tests can be performed to rule out secondary causes of osteoporosis. These typically include measurements of calcium, vitamin D, phosphate, parathyroid hormone, and bone turnover markers.

X-Ray Imaging

X-rays of the spine can reveal existing vertebral fractures, which often occur without pain and may go unnoticed. These findings are an important indicator of advanced osteoporosis.

Who Should Be Screened?

The International Osteoporosis Foundation (IOF) and national guidelines recommend bone density measurement particularly for:

  • Women aged 65 and older and men aged 70 and older
  • Younger individuals with elevated fracture risk (e.g., due to corticosteroid therapy)
  • People who have experienced a low-energy fracture (e.g., a fall from standing height)

In many countries, DXA scans are covered by health insurance when there is a medical indication.

What Happens After Early Detection?

If screening results are abnormal, the treating physician will provide individual counseling. Depending on the severity of osteoporosis, lifestyle changes (e.g., increased physical activity, calcium-rich diet, vitamin D supplementation) and/or pharmacological therapies (e.g., bisphosphonates, denosumab) are recommended to reduce fracture risk.
