What is Sudden Infant Death Syndrome?

Sudden Infant Death Syndrome (SIDS), also known as cot death or crib death, is the sudden, unexplained death of an apparently healthy infant, typically occurring during sleep. It is one of the leading causes of death in infants between one month and one year of age, with the highest incidence between two and four months of life. Despite decades of research, the exact mechanism behind SIDS is not yet fully understood.

Causes and Risk Factors

SIDS is believed to result from a combination of factors rather than a single cause. The widely accepted Triple Risk Model proposes that SIDS occurs when three conditions overlap:

  • An underlying vulnerability in the infant (e.g., immature brainstem or autonomic nervous system)
  • A critical developmental period (typically the first months of life)
  • An external environmental trigger during sleep

Known risk factors include:

  • Placing the infant to sleep on the stomach or side
  • Soft bedding, pillows, or stuffed animals in the sleep space
  • Maternal smoking during pregnancy or exposure to secondhand smoke after birth
  • Parental alcohol or drug use
  • Premature birth or low birth weight
  • Bed-sharing with parents (co-sleeping)
  • Male sex of the infant
  • Socioeconomic disadvantage

Symptoms and Recognition

By definition, SIDS occurs without warning signs. The infant appears healthy prior to death. Some parents have reported mild respiratory infections or changes in sleep patterns in the days before, but these observations are not reliable predictors. There is no known way to identify an infant at immediate risk of SIDS.

Diagnosis

SIDS is a diagnosis of exclusion, meaning it can only be established after death and after all other possible causes have been ruled out. A thorough post-mortem examination (autopsy), a review of the death scene, and a complete medical history are required. Only when no other explanation is found is the diagnosis of SIDS assigned.

Prevention and Safety Measures

While SIDS cannot be entirely prevented, evidence-based measures can significantly reduce the risk:

  • Back to sleep: Always place infants on their backs to sleep – never on the stomach or side.
  • Firm sleep surface: Use a firm, flat mattress without pillows, loose bedding, or soft toys.
  • Room-sharing, not bed-sharing: The infant should sleep in their own cot in the same room as the parents for at least the first six months.
  • Smoke-free environment: No smoking during pregnancy or near the infant after birth.
  • Appropriate room temperature: Keep the sleeping area between 16 and 20 degrees Celsius.
  • Breastfeeding: Breastfeeding is associated with a reduced SIDS risk.
  • Pacifier use: Offering a pacifier at sleep time has been shown to reduce SIDS risk.
  • Vaccinations: Keeping up with recommended vaccinations is associated with a lower SIDS risk.

Coping with Loss

The sudden death of an infant is a profoundly traumatic event for the entire family. Parents, siblings, and grandparents all require compassionate support and grief counseling. Organizations such as the First Candle foundation in the United States and similar groups worldwide provide resources and support for bereaved families.
