The term protopathic derives from the Greek words protos (= first, original) and pathos (= suffering, sensation). It describes a primitive, non-specific form of sensory perception -- particularly pain sensation -- that is considered phylogenetically older and more evolutionarily ancient than its counterpart, epikritische (epicritic) sensitivity.
The concept was largely established by the British neurologist Henry Head, who in the early 20th century, together with his colleague Gordon Holmes, distinguished two fundamental types of cutaneous sensitivity: protopathic and epicritic sensibility.
Understanding the distinction between protopathic and epicritic sensibility is key to grasping the concept:
At the neuroanatomical level, protopathic signals are primarily transmitted via the spinothalamic tract (anterolateral pathway of the spinal cord) to the brain. This pathway is phylogenetically older and projects mainly into more ancient brain structures such as the thalamus and the limbic system, which accounts for the emotional component of protopathic pain.
Epicritic sensibility, in contrast, is transmitted via the dorsal columns of the spinal cord (medial lemniscus pathway) and is more strongly integrated into the neocortex (cerebral cortex).
The term protopathic has practical significance in clinical neurology and pain medicine. It is used when:
Patients who have exclusively protopathic perception often describe their pain as burning, dull, or difficult to pinpoint. This type of pain is frequently associated with a strong emotional component because it is processed through the limbic system. It can be experienced as particularly distressing, even though precise localization of the pain stimulus is not possible.
In modern pain research, the concept has been further developed: a distinction is now drawn between fast (first) pain (mediated by A-delta fibers, sharp and well-localized) and slow (second) pain (mediated by C-fibers, burning and diffuse) -- the latter closely corresponding to the protopathic concept.
In broader medical usage, protopathic is also employed as a general adjective to describe phenomena that are primary, original, or undifferentiated -- i.e., that cannot be attributed to a specific, finely defined cause. This is to be distinguished from deuteropathic (= secondary, arising as a consequence of another disease).
