What is Shiatsu?

Shiatsu (Japanese for “finger pressure”) is a traditional Japanese bodywork therapy rooted in Chinese medicine. Practitioners apply pressure using thumbs, fingers, palms, and sometimes elbows or knees to specific points and energy pathways (meridians) along the body. The goal is to harmonize the flow of Ki – the Japanese concept of life energy – and to activate the body's natural self-healing abilities.

History and Origins

Shiatsu evolved in early 20th-century Japan from the traditional Japanese massage technique Anma and the classical Chinese practice of acupressure. Tokujiro Namikoshi is widely regarded as the founder of modern Shiatsu and established the first Shiatsu institute in Japan in 1940. His student Shizuto Masunaga later developed Zen Shiatsu, which places greater emphasis on the meridian system. Since the 1970s, Shiatsu has spread worldwide and is now practiced in many countries as a complementary therapy.

Mechanism of Action

From a traditional perspective, Shiatsu works by regulating the flow of Ki (life energy) through the meridian channels. Blockages or imbalances in this energy flow are considered the root cause of physical and emotional complaints. Targeted pressure is believed to dissolve these blockages and restore energetic balance.

From a modern, Western perspective, Shiatsu may work through the following mechanisms:

  • Stimulation of the parasympathetic nervous system, promoting relaxation
  • Improvement of local blood circulation
  • Reduction of muscle tension and pain
  • Release of endorphins and other neurotransmitters
  • Support of the lymphatic system

What to Expect During a Session

A typical Shiatsu session lasts between 60 and 90 minutes. The client usually lies fully clothed on a soft floor mat (futon). The practitioner often begins with a short health consultation and then assesses the condition of the meridians by gently palpating the abdomen (Hara diagnosis). The treatment itself involves gentle to moderate pressure, stretches, and passive joint movements. Shiatsu is generally a relaxing and pleasant experience, although mild pressure sensations may occur in some areas.

Scientific Evidence

The scientific evidence base for Shiatsu is still developing. Several studies and systematic reviews suggest positive effects on stress, pain, and sleep quality. Shiatsu is generally considered safe when performed by trained practitioners. It is not regarded as a replacement for conventional medical treatment but rather as a complementary approach.

Contraindications

In certain situations, Shiatsu should be avoided or used only after consulting a physician:

  • Acute inflammation, fever, or infectious disease
  • Thrombosis or severe vascular conditions
  • Skin conditions or open wounds in the treatment area
  • Bone metastases or severe osteoporosis
  • Early stages of pregnancy (only with a specially trained practitioner)
