What is SIDS?

SIDS stands for Sudden Infant Death Syndrome, also known as cot death or crib death. It refers to the sudden, unexpected, and unexplained death of an apparently healthy infant, typically occurring during sleep. Even after a thorough post-mortem examination and a detailed review of the circumstances, no cause of death can be identified. SIDS most commonly occurs between the ages of two and four months and is one of the leading causes of death in infants under one year of age in high-income countries.

Causes and Risk Factors

The exact cause of SIDS remains unknown. Researchers believe it results from a combination of factors described by the Triple Risk Model, which identifies three overlapping areas:

  • Critical developmental window: Infants between approximately 2 and 4 months of age are in a particularly vulnerable stage of development.
  • Underlying vulnerability: Some infants may have subtle abnormalities in brainstem regions responsible for regulating breathing, heart rate, and arousal during sleep.
  • External stressors: Environmental factors such as prone sleeping position, overheating, or soft bedding may act as triggers in vulnerable infants.

Identified risk factors include:

  • Sleeping in a prone (face down) or side position
  • Soft or unstable sleep surfaces (e.g., sofas, waterbeds, adult beds)
  • Maternal smoking during pregnancy or postnatal exposure to secondhand smoke
  • Premature birth or low birth weight
  • Male sex (boys are more frequently affected)
  • Bed-sharing with parents or other adults
  • Parental alcohol or drug use
  • Lower socioeconomic status

Diagnosis

SIDS is a diagnosis of exclusion, meaning it is only confirmed after all other possible causes of death have been ruled out. This process includes:

  • Complete autopsy
  • Thorough investigation of the sleep environment
  • Review of the infant's complete medical history
  • Exclusion of infections, metabolic disorders, cardiac conditions, and signs of injury

Only when no explanation can be found after all these investigations is SIDS recorded as the cause of death.

Prevention

While SIDS cannot be completely prevented, evidence-based measures can significantly reduce the risk:

  • Back to sleep: Always place infants on their back to sleep – never on their front or side.
  • Firm, flat sleep surface: Use a firm, flat mattress in a safety-approved cot or crib, free from pillows, loose blankets, bumpers, and soft toys.
  • Smoke-free environment: Avoid smoking during pregnancy and keep the infant away from secondhand smoke after birth.
  • Separate sleep space: Infants should sleep in their own cot in the same room as the parents, not in the parental bed.
  • Breastfeeding: Breastfeeding has been associated with a reduced risk of SIDS.
  • Pacifier use: Offering a pacifier at the start of each sleep period may reduce SIDS risk.
  • Room temperature: Keep the sleeping environment at a comfortable temperature (around 16–18 °C or 60–65 °F) and avoid overheating.
  • Vaccinations: Keeping up with the recommended immunization schedule is advised and has not been shown to increase SIDS risk.

Support for Bereaved Families

The sudden loss of an infant to SIDS is one of the most devastating experiences a family can face. Professional grief counseling, psychological support, and peer support groups play a vital role in helping parents and siblings cope. Organizations such as the Lullaby Trust (UK), First Candle (US), and similar national charities provide resources, helplines, and community support for bereaved families.
