Things worth knowing about "Spondylolysis"

Spondylolysis is a stress fracture or defect in the pars interarticularis of a vertebra, most commonly in the lower back. It frequently causes lower back pain in young athletes.

What is Spondylolysis?

Spondylolysis is a defect or stress fracture in a specific part of the vertebra called the pars interarticularis -- a small bridge of bone connecting the upper and lower joints of a vertebra. It most commonly affects the fifth lumbar vertebra (L5) in the lower back. The condition is one of the most frequent causes of lower back pain in children, adolescents, and physically active adults.

Causes

Several factors can contribute to the development of spondylolysis:

  • Repetitive stress and overuse: Activities that repeatedly hyperextend or rotate the spine -- such as gymnastics, weightlifting, football, or wrestling -- place excessive stress on the pars interarticularis, potentially causing a fatigue fracture.
  • Genetic predisposition: A familial tendency has been observed, with some individuals having naturally thinner bone in this region of the vertebra.
  • Congenital defect: In rare cases, the defect is present from birth due to abnormal bone development.
  • Trauma: A direct injury to the spine can also result in this type of fracture.

Symptoms

Many people with spondylolysis experience no symptoms at all, and the condition is discovered incidentally during imaging for other reasons. When symptoms do occur, they may include:

  • Dull or sharp pain in the lower back, often worsening with activity
  • Increased pain when bending backward (spinal extension)
  • Pain that radiates to the buttocks or upper thighs
  • Muscle tightness or stiffness in the lower back
  • In rare cases, numbness, tingling, or weakness in the legs, especially if a vertebral slip (spondylolisthesis) has developed

Diagnosis

Diagnosis of spondylolysis involves a combination of physical examination and imaging studies:

  • X-rays: The initial imaging study of choice; oblique views may show the classic Scottie dog sign, where the defect appears as a collar or break in the neck of the dog-shaped vertebral outline.
  • Computed Tomography (CT scan): Provides detailed images of the bony defect and is useful for assessing the severity and chronicity of the fracture.
  • Magnetic Resonance Imaging (MRI): Particularly valuable for early detection of stress injuries and for evaluating surrounding soft tissues and nerve structures.
  • Bone scintigraphy (bone scan): Can detect active (acute) stress fractures and is used in ambiguous cases.

Treatment

Treatment depends on the severity of the condition, the age of the patient, and the degree of symptoms:

Conservative Treatment

  • Rest and activity modification: Temporary reduction or cessation of sports activities is essential, especially for acute stress fractures.
  • Physical therapy: Targeted exercises to strengthen the core and back muscles, along with stretching and posture correction.
  • Pain management: Non-steroidal anti-inflammatory drugs (NSAIDs) may be used to relieve pain and reduce inflammation.
  • Bracing: A lumbar brace or orthotic support may be recommended, particularly in adolescents with active fractures.

Surgical Treatment

Surgery is only required in a minority of cases and is considered when:

  • Conservative treatment has failed after several months
  • Neurological deficits are present
  • A progressive vertebral slip (spondylolisthesis) accompanies the condition

Surgical options include direct repair of the pars defect (pars repair) or, in more advanced cases, spinal fusion (spondylodesis) of the affected vertebral segment.

Prognosis

The prognosis for spondylolysis is generally favorable when the condition is identified and managed promptly. Many patients -- including young athletes -- are able to return to full sporting activity following dedicated physical therapy and appropriate rest. Regular medical follow-up is important to monitor for the possible development of vertebral slippage.
