Things worth knowing about "Trabecular Bone"

Trabecular bone is the spongy inner structure of bones, essential for mechanical stability and bone metabolism. It plays a key role in calcium regulation and blood cell production.

What Is Trabecular Bone?

Trabecular bone – also known as cancellous bone or spongy bone – is the porous, lattice-like inner layer of human bone. It consists of a three-dimensional network of small bony struts called trabeculae, which provide high structural strength while keeping the overall weight of the skeleton low.

In contrast to cortical bone (compact bone), which forms the dense outer shell of bones, trabecular bone is much more loosely organized. It accounts for approximately 20% of total bone mass but contributes disproportionately to bone metabolism due to its large surface area.

Location in the Body

Trabecular bone is found primarily in:

  • The ends of long bones (e.g., femur, tibia)
  • The vertebral bodies of the spine
  • Flat bones such as the pelvis and sternum
  • Short bones such as the carpal and tarsal bones

In these regions, it fulfills important mechanical and biological functions.

Functions of Trabecular Bone

Mechanical Function

The trabecular network is oriented to absorb and distribute mechanical forces optimally. This makes bone resistant to compressive, tensile, and bending loads without adding unnecessary weight.

Metabolic Function

Due to its large internal surface area, trabecular bone is far more metabolically active than cortical bone. It responds rapidly to hormonal changes, calcium and phosphate regulation, and mechanical loading.

Blood Cell Production

The spaces within the trabecular network contain red bone marrow, which is responsible for the production of blood cells – a process known as hematopoiesis.

Bone Remodeling

Trabecular bone is continuously renewed through a process called bone remodeling, which is regulated by two main cell types:

  • Osteoblasts: Cells that build new bone tissue
  • Osteoclasts: Cells that break down old bone tissue

This process allows the body to adapt bone structure to changing mechanical demands and to repair microscopic damage. Trabecular bone is remodeled approximately 8 times faster than cortical bone due to its extensive surface area.

Clinical Relevance

Osteoporosis

The most well-known condition affecting trabecular bone is osteoporosis. In this disease, bone density decreases, the trabeculae become thinner and may disconnect, significantly increasing the risk of fractures – particularly at the spine, femoral neck, and wrist.

Bone Mineral Density Measurement (DXA)

The dual-energy X-ray absorptiometry (DXA) scan measures bone mineral density including trabecular bone and is a key diagnostic tool for osteoporosis and related bone diseases.

Other Conditions

Trabecular bone is also affected in the following conditions:

  • Osteomalacia: Softening of bone due to vitamin D deficiency
  • Paget's disease of bone: Disordered bone remodeling
  • Bone metastases: Spread of cancer cells into bone tissue

Factors Influencing Trabecular Bone Health

Several factors affect the density and integrity of trabecular bone:

  • Nutrition: Adequate calcium and vitamin D are essential for bone formation
  • Physical activity: Weight-bearing and resistance exercises stimulate bone building
  • Hormones: Estrogen and testosterone protect bone; their decline (e.g., during menopause) accelerates bone loss
  • Age: Bone density naturally decreases with aging
  • Medications: Long-term use of corticosteroids can reduce bone density
