Transient Elastography – Measuring Liver Stiffness

What is Transient Elastography?

Transient elastography (widely known by the brand name FibroScan®) is a modern, non-invasive diagnostic method that measures the stiffness (elasticity) of liver tissue. Since increasing liver stiffness is typically associated with progressive liver fibrosis – an abnormal accumulation of scar tissue in the liver – this technique provides valuable information about the health of the liver. It is used worldwide as a standard method in hepatology and, in many cases, replaces the more burdensome liver biopsy.

Principle of Operation

During transient elastography, a specialized ultrasound probe is placed on the skin over the right side of the ribcage. The device transmits a mild, controlled mechanical vibration along with an accompanying ultrasound pulse into the liver tissue. Based on the speed at which this shear wave travels through the tissue, the system calculates tissue stiffness in kilopascals (kPa).

  • Soft, healthy liver tissue transmits the shear wave slowly.
  • Hardened, fibrotic tissue transmits the shear wave more rapidly.
  • Higher kPa values indicate advanced fibrosis or liver cirrhosis.

Modern devices can also measure the CAP value (Controlled Attenuation Parameter), which quantifies the fat content of the liver (steatosis).

When is Transient Elastography Used?

The examination is primarily used in conditions where chronic liver damage is expected or needs to be monitored:

  • Chronic hepatitis B and C
  • Alcoholic liver disease (ALD)
  • Non-alcoholic fatty liver disease (NAFLD/NASH)
  • Primary biliary cholangitis (PBC) and other cholestatic liver diseases
  • Hemochromatosis and other metabolic liver disorders
  • Follow-up monitoring after liver transplantation

How the Examination is Performed

The procedure is painless and typically takes only a few minutes. The patient lies relaxed on their back with the right arm raised behind the head. A gel is applied to the skin to ensure good contact with the probe. The device automatically performs multiple measurements (typically 10 valid readings), from which a median value is calculated. Fasting prior to the examination may be recommended depending on the clinical protocol, as recent meals can temporarily increase stiffness values.

Interpretation of Results

Results are interpreted according to clinically validated thresholds, which may vary depending on the underlying liver disease:

  • Below 7 kPa: Typically no or minimal fibrosis (F0–F1)
  • 7–9.5 kPa: Moderate fibrosis (F2)
  • 9.5–12.5 kPa: Advanced fibrosis (F3)
  • Above 12.5–13 kPa: Indicative of liver cirrhosis (F4)

These thresholds may vary depending on the etiology of the liver disease and the device used. Results must always be evaluated by a physician in the context of the overall clinical picture.

  • Simultaneous measurement of liver fat content possible (CAP)

Limitations

  • Elevated body mass index (obesity) can reduce measurement quality (special XL probes are available)
  • Ascites (fluid in the abdominal cavity) can make measurement impossible
  • Acute liver inflammation (significantly elevated transaminases) can temporarily increase stiffness and lead to overestimation of fibrosis
  • Not a substitute for liver biopsy in unclear findings or suspected rare liver conditions
