General anaesthesia (also spelled general anesthesia) is a medically controlled state of total unconsciousness. During general anaesthesia, awareness, pain sensation, reflexes, and muscle tone are completely suppressed. It is primarily used for major surgical procedures where local or regional anaesthesia is insufficient or unsuitable.
General anaesthesia is achieved through a combination of drugs that act on the central nervous system to produce four essential components:
The medications used are administered either intravenously (directly into the bloodstream) or as inhaled gases through the airways. Commonly used agents include propofol, sevoflurane, fentanyl, and rocuronium.
Before the procedure, the patient undergoes a detailed consultation with an anaesthesiologist. Medical history, current medications, allergies, and overall health status are reviewed. The patient is typically required to fast for several hours to minimise the risk of aspiration (inhalation of stomach contents).
Anaesthesia is usually induced via an intravenous injection of a sedative agent such as propofol. Loss of consciousness occurs within seconds. Analgesics and muscle relaxants are then administered, and ventilation is secured using an endotracheal tube or a laryngeal mask airway.
During surgery, anaesthesia is maintained using an inhaled anaesthetic agent (e.g., sevoflurane) or a continuous intravenous infusion. The anaesthesiologist continuously monitors heart rate, blood pressure, oxygen saturation, and respiratory function throughout the procedure.
At the end of the procedure, anaesthetic agents are discontinued. The patient awakens in a dedicated recovery room, where vital signs are closely monitored by medical staff until the patient is stable and fully oriented.
General anaesthesia is used for a wide range of procedures, including:
General anaesthesia is considered very safe with modern medications and monitoring technology. However, side effects can occur:
Individual risk depends significantly on age, pre-existing conditions, and the type of procedure. Serious complications are extremely rare in otherwise healthy patients.
Children are more sensitive to anaesthetic agents and require carefully adjusted dosages. Elderly patients face a higher risk of postoperative cognitive dysfunction. In pregnant patients, general anaesthesia is avoided where possible; when necessary, special protective measures are taken to safeguard the unborn child.
