YKL-40, also known as Chitinase-3-like Protein 1 (CHI3L1), is a secreted glycoprotein belonging to the family of chitinase-like proteins. The name is derived from the three amino acids at the N-terminus of the protein — Tyrosine (Y), Lysine (K), and Leucine (L) — combined with its molecular weight of 40 kilodaltons. Although YKL-40 shares structural similarities with chitinases, it does not possess enzymatic chitin-cleaving activity.
The protein is produced by various cell types, including macrophages, neutrophils, chondrocytes, synovial fibroblasts, and tumor cells. It plays an important role in inflammatory processes, tissue remodeling, and cell proliferation.

Elevated YKL-40 levels in blood serum or cerebrospinal fluid (CSF) are associated with a wide range of diseases and serve as a diagnostic or prognostic biomarker:
Elevated serum YKL-40 levels have been detected in various cancers, including glioblastoma, breast, lung, colorectal, and ovarian cancer. Higher levels frequently correlate with poorer prognosis and shorter survival. YKL-40 is therefore being investigated as a monitoring and prognostic biomarker in oncology.
In asthma and chronic obstructive pulmonary disease (COPD), YKL-40 levels are elevated and reflect the severity of the disease as well as inflammatory remodeling processes in the airways. In asthma, YKL-40 is associated with a Th2-independent inflammatory phenotype.
Elevated YKL-40 levels have been measured in the CSF and serum of patients with multiple sclerosis, Alzheimer's disease, amyotrophic lateral sclerosis (ALS), and other neurodegenerative conditions. It is considered a marker of neuroinflammatory and neurodegenerative processes, as well as of astrocyte and microglia activation.
YKL-40 correlates with the extent of liver fibrosis and cirrhosis and is used for non-invasive assessment of fibrosis severity in chronic liver diseases such as hepatitis B, hepatitis C, and non-alcoholic fatty liver disease (NAFLD).
In rheumatoid arthritis and other inflammatory rheumatic conditions, YKL-40 levels are elevated and correlate with disease activity.
YKL-40 is typically measured using an ELISA (Enzyme-Linked Immunosorbent Assay) in serum, plasma, or CSF. Reference values can vary depending on the laboratory, age, and sex. YKL-40 levels naturally increase with age, so age-corrected reference ranges should be applied during interpretation.
YKL-40 is not a disease-specific biomarker, meaning elevated values must always be interpreted in the clinical context and in combination with further diagnostic investigations.
Research into YKL-40 is an active area of medical science. YKL-40 is also being discussed as a potential therapeutic target — particularly in oncology and inflammatory lung diseases, where inhibition of the protein may positively influence the course of disease. Relevant clinical trials are currently underway.
