What is the 5 Rights Rule?

The 5 Rights Rule (also known as the Five Rights of Medication Administration) is a core safety principle used in nursing and clinical practice to prevent medication errors. Before administering any medication, healthcare professionals are required to verify five key checkpoints to ensure the safety of the patient and the accuracy of the treatment.

The Five Checkpoints

The 5 Rights Rule consists of the following verification steps:

Right Patient: Is the medication being given to the correct person? Patient identity must be confirmed, for example by checking a wristband or asking the patient to state their name and date of birth.
Right Medication: Is the correct drug being administered as prescribed? Look-alike and sound-alike medications must be carefully distinguished.
Right Dose: Is the correct amount being given? Both overdoses and underdoses can have serious consequences for the patient.
Right Time: Is the medication being administered at the correct time? Some medications must be given at specific intervals or at particular times of day to be effective.
Right Route: Is the medication being given via the correct route of administration, such as oral, intravenous, subcutaneous, or intramuscular?

Extensions of the Rule

Modern clinical practice has expanded the original 5 Rights to include additional safety checks. The 6 Rights or even the 10 Rights of Medication Administration incorporate further considerations such as proper documentation, patient education, checking for allergies, medication expiry dates, and the right to refuse. These extensions reflect the increasing emphasis on patient safety across healthcare settings.
