.Acne vulgaris: Chronic inflammatory skin disease with multifactorial causes
Acne vulgaris is the most common skin disease worldwide and mainly affects adolescents during puberty, but can also persist or recur in adulthood. It is characterised by blackheads (comedones), inflammatory papules, pustules and, in severe cases, lumps or cysts, which mainly appear on the face, back and chest. The disease is caused by a combination of several factors: increased sebum production, a keratinisation disorder of the sebaceous gland outlets, bacterial colonisation with Cutibacterium acnes and an excessive inflammatory reaction of the skin.
Hormonal influences - in particular elevated androgen levels - play a central role in the development of the condition. Genetic predisposition, psychological stress, diet (e.g. high consumption of sugar, dairy products or highly processed foods) and the composition of the skin microbiome can also influence acne. Acne vulgaris is not only a cosmetic problem, but can also have a negative impact on self-esteem and leave scars in severe cases.
The treatment depends on the severity and individual characteristics. Mild forms respond well to topical active ingredients such as salicylic acid, benzoyl peroxide, retinoids or azelaic acid. For moderate to severe cases, systemic therapies such as antibiotics, hormone preparations or isotretinoin are used. In addition, holistic approaches are increasingly being researched that focus on microbiome regulation, gut health and an anti-inflammatory diet.
