Die Addison-Krankheit (Morbus Addison) ist eine seltene Erkrankung, bei der die Nebennierenrinde zu wenig Cortisol und häufig auch Aldosteron produziert.
Die Ursachen der Addison-Krankheit sind meist autoimmun bedingt. Seltener sind sie infektiös (z. B. Tuberkulose) oder genetisch bedingt. Auch Medikamente können die Krankheit auslösen.
Da Cortisol ein lebenswichtiges Stress- und Stoffwechselhormon ist, führt der Mangel zu unspezifischen, aber ausgeprägten Symptomen wie chronischer Müdigkeit, Muskel- und Gelenkschmerzen, Gewichtsverlust, niedrigem Blutdruck, Salzhunger und verstärkter Hautpigmentierung. In akuten Fällen kann eine Addison-Krise mit Schock, Verwirrtheit und Koma auftreten – ein Notfall, der sofortige Cortisolgabe erfordert.
Die Diagnose erfolgt über Hormonmessung (ACTH-Stimulationstest) und Bildgebung. Die Behandlung besteht aus einer lebenslangen Substitutionstherapie mit Glukokortikoiden (z. B. Hydrocortison) und bei Bedarf Mineralokortikoiden. Bei Infekten, Operationen oder Stresssituationen ist eine Dosisanpassung essenziell, um eine Krise zu vermeiden.
