What is Adherence Therapy?

Adherence therapy is a structured, patient-centred therapeutic approach designed to strengthen a patient´s willingness and ability to consistently follow their prescribed medical treatment. The term adherence refers to the degree to which a person´s behaviour aligns with the agreed recommendations of their healthcare provider. Unlike the older concept of compliance, adherence emphasises the patient´s active role and partnership in the treatment process.

Adherence therapy was originally developed for people with mental health conditions, particularly patients with schizophrenia who frequently discontinue their antipsychotic medication. Today, it is applied across a wide range of chronic conditions, including diabetes mellitus, hypertension, HIV infection, and depression.

Goals of Adherence Therapy

The primary goal is to achieve a sustained improvement in treatment adherence. Specifically, adherence therapy aims to:

  • openly address and resolve ambivalence towards medication
  • improve the patient´s understanding of their illness and treatment
  • reduce negative beliefs and fears about medications
  • strengthen the patient´s sense of self-efficacy and personal responsibility
  • foster a strong and trusting therapeutic relationship between patient and clinician

Methods and Approach

Adherence therapy integrates elements from several evidence-based psychotherapeutic methods:

Motivational Interviewing

Motivational interviewing is a core element of adherence therapy. The therapist engages with the patient as an equal partner, explores personal attitudes towards treatment, and helps the patient to identify their own intrinsic motivations for continuing therapy without applying pressure.

Cognitive Behavioural Therapy

Elements of cognitive behavioural therapy (CBT) are used to identify and modify dysfunctional thought patterns related to the illness or medication. Misconceptions, such as the belief that medication causes more harm than good, are addressed and corrected in an evidence-based, collaborative manner.

Psychoeducation

Through targeted psychoeducation, patients receive clear, evidence-based information about their condition, how their medication works, and what side effects to expect. This knowledge empowers patients to make informed decisions about their treatment.

Problem-Solving Strategies

Practical barriers to medication adherence, such as forgetfulness, side effects, or organisational challenges, are identified together with the patient, and concrete problem-solving strategies are developed.

Structure of Adherence Therapy

Adherence therapy is typically delivered as a brief intervention consisting of 4 to 8 sessions. It can be provided in individual or group formats. The therapy generally follows these phases:

  • Phase 1: Exploration of the patient´s attitudes towards medication and their illness history
  • Phase 2: Collaborative analysis of the perceived pros and cons of treatment
  • Phase 3: Working through ambivalence and negative beliefs
  • Phase 4: Development of an individualised plan to maintain long-term adherence

Areas of Application

Adherence therapy is used in a wide variety of conditions that require long-term medication use:

  • Mental health conditions (schizophrenia, bipolar disorder, depression)
  • Cardiovascular diseases (hypertension, heart failure)
  • Metabolic disorders (type 2 diabetes mellitus)
  • Infectious diseases (HIV, tuberculosis)
  • Chronic inflammatory conditions (rheumatoid arthritis)

Effectiveness and Evidence

Several clinical studies support the effectiveness of adherence therapy. Particularly in patients with schizophrenia, significant improvements in medication adherence and reductions in relapse rates have been demonstrated. However, a Cochrane review found that effects are moderate and not uniformly consistent across all patient groups, highlighting the importance of individually tailored application.
