What is Alprazolam?

Alprazolam is a benzodiazepine -- a class of central nervous system depressants with sedative, anxiolytic (anti-anxiety), muscle-relaxant, and anticonvulsant properties. It is one of the most widely prescribed psychotropic medications in the world and is available only by prescription. Common brand names include Xanax and Niravam.

Indications

Alprazolam is indicated for the treatment of:

  • Anxiety disorders -- particularly generalized anxiety disorder (GAD)
  • Panic disorder -- with or without agoraphobia
  • Short-term relief of severe anxiety symptoms
  • Anxiety associated with depressive episodes

Mechanism of Action

Alprazolam exerts its effects by enhancing the activity of gamma-aminobutyric acid (GABA), the main inhibitory neurotransmitter in the brain. It binds to the GABA-A receptor, increasing the frequency of chloride ion channel opening. This leads to hyperpolarization of the neuron and a reduction in neuronal excitability, producing calming and anxiolytic effects.

Dosage

Dosage is individualized based on the condition being treated and the patient profile. General guidelines include:

  • Anxiety disorders: 0.25–0.5 mg three times daily
  • Panic disorder: up to 10 mg per day in severe cases under close medical supervision
  • Extended-release formulations allow for once-daily dosing

Alprazolam should generally be used for short-term treatment only (typically 4–12 weeks) to reduce the risk of dependence.

Side Effects

Like all benzodiazepines, alprazolam can cause side effects. The most common include:

  • Drowsiness and sedation
  • Dizziness and impaired coordination
  • Memory and concentration difficulties
  • Dry mouth
  • Changes in libido

Serious Side Effects

  • Physical and psychological dependence with prolonged use
  • Withdrawal symptoms upon abrupt discontinuation (e.g., insomnia, tremors, seizures)
  • Respiratory depression -- particularly when combined with alcohol or other CNS depressants
  • Paradoxical reactions (agitation, aggression) -- more common in elderly patients

Drug Interactions

Alprazolam interacts with several substances and medications:

  • Alcohol: Significantly enhances CNS depression -- this combination can be life-threatening
  • Opioids: Increased risk of respiratory depression and overdose
  • CYP3A4 inhibitors (e.g., ketoconazole, erythromycin): Increase alprazolam plasma levels
  • CYP3A4 inducers (e.g., rifampin): Reduce the effectiveness of alprazolam

Contraindications

Alprazolam should not be used in patients with:

  • Known hypersensitivity to benzodiazepines
  • Severe obstructive sleep apnea
  • Severe respiratory insufficiency
  • Myasthenia gravis (a neuromuscular disorder causing muscle weakness)
  • Pregnancy (especially the first trimester) and breastfeeding

Dependence and Discontinuation

Alprazolam carries a high potential for dependence. Both physical and psychological dependence can develop after only a few weeks of regular use. Discontinuation should always be done gradually and under medical supervision to minimize withdrawal symptoms. Patients with a history of substance use disorders are at particularly elevated risk and should be monitored closely.
