What is Ambulatory Blood Pressure Monitoring?

Ambulatory blood pressure monitoring (ABPM) is a diagnostic method in which a patient´s blood pressure is automatically recorded over a period of 24 hours during normal daily activities. Unlike a single measurement taken in a doctor´s office, ABPM provides a comprehensive picture of blood pressure behavior throughout the day, at night, and during various activities and rest periods.

How Does Ambulatory Blood Pressure Monitoring Work?

The patient wears a small portable device attached to the body and an inflatable cuff on the upper arm. The device automatically measures blood pressure at regular intervals – typically every 15 to 30 minutes during the day and every 30 to 60 minutes at night. All readings are stored and later evaluated by the physician.

When is Ambulatory Blood Pressure Monitoring Used?

  • Suspected hypertension: Confirmation or exclusion of persistently elevated blood pressure
  • White coat hypertension: Identifying blood pressure that is elevated only in the clinical setting but normal in daily life
  • Masked hypertension: Detecting elevated blood pressure that appears normal during office visits
  • Treatment monitoring: Assessing the effectiveness of antihypertensive medications
  • Nocturnal dipping: Evaluating the nighttime blood pressure drop, which is an important cardiovascular risk marker

Understanding the Readings

According to the guidelines of the European Society of Cardiology (ESC) and the European Society of Hypertension (ESH), the following thresholds apply for ABPM:

  • Daytime average: below 135/85 mmHg
  • Nighttime average: below 120/70 mmHg
  • 24-hour average: below 130/80 mmHg

Values above these thresholds may indicate hypertension that requires treatment.

Advantages Over Single Measurements

ABPM offers several advantages compared to isolated office blood pressure readings:

  • Detection of blood pressure variability over the course of the day and night
  • Avoidance of misdiagnosis due to the white coat effect
  • Better assessment of overall cardiovascular risk
  • More accurate monitoring of medication effectiveness

Procedure and Preparation

The examination is largely comfortable for the patient and requires no special preparation. Normal daily activities – including work, light exercise, and sleep – can continue as usual. However, intense physical activity and bathing or showering should be avoided during the measurement period. Keeping a diary of activities, sleep, and any symptoms can help the physician interpret the results more accurately.

Risks and Side Effects

ABPM is a low-risk procedure. Some patients may find the inflation of the cuff uncomfortable, particularly during nighttime measurements. Mild skin irritation under the cuff can occasionally occur. No serious complications are associated with this method.
