What is Amlodipine?

Amlodipine is a medication belonging to the class of calcium channel blockers (also called calcium antagonists). It is primarily used to treat high blood pressure (arterial hypertension) and angina pectoris (chest pain associated with coronary artery disease). Amlodipine is one of the most widely prescribed cardiovascular medications in the world and has been a well-established treatment for decades.

Indications (Uses)

Amlodipine is prescribed for:

  • Arterial hypertension – to lower persistently elevated blood pressure
  • Stable angina pectoris – to prevent chest pain during physical exertion caused by coronary artery disease
  • Vasospastic angina (Prinzmetal angina) – to relieve chest pain caused by coronary artery spasms

Mechanism of Action

Amlodipine selectively blocks voltage-dependent L-type calcium channels in the smooth muscle of blood vessels and in cardiac muscle cells. Calcium is essential for muscle cell contraction. By blocking these channels, less calcium enters the muscle cells of the vessel walls, causing the blood vessels to relax and widen (vasodilation). This results in:

  • Reduction of peripheral vascular resistance
  • Lowering of blood pressure
  • Improved blood flow to the heart
  • Reduced oxygen demand of the heart muscle

Amlodipine acts predominantly on peripheral arteries and has a comparatively low effect on the heart muscle itself (low negative inotropic effect). Its long half-life of 35 to 50 hours allows for once-daily dosing.

Dosage and Administration

Amlodipine is taken orally as a tablet. The usual dosage is:

  • Adults: 5 mg once daily as a starting dose; may be increased to 10 mg once daily if needed
  • Elderly patients and those with hepatic impairment: A lower starting dose is generally recommended
  • Children (6–17 years): 2.5 mg to 5 mg once daily under medical supervision

Tablets may be taken with or without food. Taking the medication at the same time each day helps maintain consistent blood pressure control.

Side Effects

Like all medications, amlodipine can cause side effects. The most common include:

  • Edema (fluid retention, particularly in the ankles and legs) – the most frequent side effect
  • Headache and dizziness
  • Flushing and feeling of warmth
  • Fatigue
  • Palpitations (rapid or irregular heartbeat)
  • Nausea and abdominal discomfort

Rare but serious side effects include severe low blood pressure (hypotension), cardiac arrhythmias, or allergic reactions. Patients experiencing persistent or severe symptoms should seek medical attention promptly.

Drug Interactions

Amlodipine can interact with other medications. Important interactions include:

  • Other antihypertensive agents (enhanced blood pressure-lowering effect)
  • Simvastatin – amlodipine can increase simvastatin blood levels (dose adjustment recommended)
  • Ciclosporin – elevated ciclosporin levels are possible
  • Grapefruit juice – can slow the breakdown of amlodipine and intensify its effect

Contraindications

Amlodipine should not be used in patients with:

  • Known hypersensitivity to amlodipine or other calcium channel blockers
  • Severe low blood pressure (severe hypotension)
  • Cardiogenic shock
  • Unstable angina pectoris (except vasospastic angina)
  • Hemodynamically significant aortic stenosis

Special Precautions

Amlodipine is not recommended during pregnancy or breastfeeding, as its safety has not been sufficiently established. Patients with liver disease may require dose adjustments. Discontinuing amlodipine should always be done in consultation with a physician – abrupt discontinuation may lead to a rebound increase in blood pressure.
