What is Antibody-Dependent Cellular Cytotoxicity?

Antibody-dependent cellular cytotoxicity (abbreviated ADCC) is a key defense mechanism of the human immune system. In this process, antibodies – primarily of the IgG class – bind to specific structures (antigens) on the surface of target cells, such as tumor cells or virus-infected cells. This labeling then activates specialized immune effector cells that destroy the marked target cell.

Mechanism of Action

The ADCC process can be divided into several steps:

  • Antibody binding: An antibody binds via its Fab fragment (the antigen-binding portion) to a specific antigen on the surface of the target cell.
  • Recognition by effector cells: The Fc region of the antibody (the constant portion) is recognized by the Fc receptor (FcγRIII, also known as CD16) on effector cells. The primary effector cells involved are natural killer (NK) cells, as well as monocytes, macrophages, and neutrophils.
  • Activation of effector cells: Binding of the Fc receptor to the antibody triggers a signaling cascade within the effector cell.
  • Cell destruction: Activated effector cells release cytotoxic molecules such as perforin and granzymes. Perforin forms pores in the membrane of the target cell, allowing granzymes to enter and initiate apoptosis (programmed cell death).

Biological Significance

ADCC plays a central role in the defense against viral infections, the elimination of tumor cells, and the control of parasites. It represents a critical link between the humoral immune response (antibodies) and the cellular immune response (immune cells), forming an integral part of adaptive immunity.

Clinical Relevance and Therapeutic Applications

The principle of ADCC is actively exploited in modern medicine for therapeutic purposes:

Monoclonal Antibodies in Oncology

Many therapeutic monoclonal antibodies exert their effects at least in part through ADCC. Well-known examples include:

  • Rituximab (targeting CD20 on B-cell lymphomas)
  • Trastuzumab (targeting HER2-positive breast cancer cells)
  • Cetuximab (targeting EGFR-positive tumor cells)

These antibodies bind to tumor-specific antigens and recruit NK cells and other effector cells to destroy the tumor cells.

ADCC and Antiviral Immune Response

During viral infections such as HIV or influenza, antibodies label virus-infected cells, which are then eliminated by NK cells via ADCC. The strength of the ADCC response is considered a prognostic factor in various infectious diseases.

Optimization of Antibody Therapies

Active research in biopharmaceutical development aims to enhance the ADCC activity of therapeutic antibodies through targeted modifications to the Fc region, a process known as Fc engineering. The goal is to increase binding affinity to the Fc receptor, thereby improving the efficiency of tumor cell destruction.

Distinction from Related Immune Mechanisms

ADCC should be distinguished from other antibody-mediated mechanisms:

  • Complement-dependent cytotoxicity (CDC): Here, the complement system is activated, leading directly to lysis of the target cell without the involvement of effector cells.
  • Direct apoptosis induction: Some antibodies can directly trigger cell death through receptor blockade or activation, independent of effector cells.
  • Phagocytosis: Antibody-labeled target cells can also be phagocytosed (engulfed and digested) by macrophages.
