What Is the Apgar Score?

The Apgar Score is an internationally recognized assessment system used immediately after birth to rapidly and systematically evaluate the general health of a newborn. It was developed in 1952 by American anesthesiologist Virginia Apgar and remains an essential tool in obstetrics and neonatology to this day.

The acronym APGAR also serves as a mnemonic: Appearance (skin color), Pulse (heart rate), Grimace (reflex response), Activity (muscle tone), and Respiration (breathing effort).

How Is the Apgar Score Assessed?

The score is recorded at three fixed time points after birth: at 1 minute, at 5 minutes, and at 10 minutes. Each of the five criteria is rated 0, 1, or 2 points, resulting in a maximum total score of 10 points.

Appearance (Skin Color): Blue or pale all over = 0, body pink with blue extremities = 1, completely pink = 2
Pulse (Heart Rate): No pulse = 0, below 100 beats per minute = 1, above 100 beats per minute = 2
Grimace (Reflex Response): No response to stimulation = 0, weak response = 1, vigorous cry or cough = 2
Activity (Muscle Tone): Limp = 0, some flexion of extremities = 1, active movement = 2
Respiration (Breathing): No breathing = 0, weak or irregular breathing = 1, strong cry and regular breathing = 2

Interpretation of Scores

The total score provides guidance on the condition of the newborn and the need for medical intervention:

8 to 10 points: Normal finding -- the newborn is in good condition and requires no special intervention.
5 to 7 points: Mild adaptation disorder -- close monitoring is required; simple measures such as suctioning or supplemental oxygen may be needed.
Below 5 points: Severe adaptation disorder -- immediate resuscitation and intensive medical care are necessary.

Clinical Significance and Limitations

The Apgar Score enables a rapid and reproducible initial assessment and facilitates communication among obstetricians, midwives, and pediatricians. It provides a first indication of the postnatal adaptation status of the newborn.

However, the score has clear limitations: it is not a predictor of long-term neurological development and can be influenced by prematurity, maternal sedation, anesthesia, or certain neuromuscular conditions. A low Apgar Score alone does not provide a definitive prognosis regarding the child's long-term health outcomes.

Historical Background

Virginia Apgar originally developed the scoring system to systematically assess the effects of obstetric anesthesia on newborns. Since its introduction, the Apgar Score has become a global standard in perinatal medicine and is routinely applied in virtually all maternity units worldwide.
