The Apgar Score is an internationally recognized assessment system used immediately after birth to rapidly and systematically evaluate the general health of a newborn. It was developed in 1952 by American anesthesiologist Virginia Apgar and remains an essential tool in obstetrics and neonatology to this day.
The acronym APGAR also serves as a mnemonic: Appearance (skin color), Pulse (heart rate), Grimace (reflex response), Activity (muscle tone), and Respiration (breathing effort).
The score is recorded at three fixed time points after birth: at 1 minute, at 5 minutes, and at 10 minutes. Each of the five criteria is rated 0, 1, or 2 points, resulting in a maximum total score of 10 points.
The total score provides guidance on the condition of the newborn and the need for medical intervention:
The Apgar Score enables a rapid and reproducible initial assessment and facilitates communication among obstetricians, midwives, and pediatricians. It provides a first indication of the postnatal adaptation status of the newborn.
However, the score has clear limitations: it is not a predictor of long-term neurological development and can be influenced by prematurity, maternal sedation, anesthesia, or certain neuromuscular conditions. A low Apgar Score alone does not provide a definitive prognosis regarding the child's long-term health outcomes.
Virginia Apgar originally developed the scoring system to systematically assess the effects of obstetric anesthesia on newborns. Since its introduction, the Apgar Score has become a global standard in perinatal medicine and is routinely applied in virtually all maternity units worldwide.
