What Are Appendix Symptoms?

The term appendix symptoms refers to the signs and complaints that indicate an inflammation of the vermiform appendix – medically known as appendicitis. Appendicitis is a medical emergency that requires prompt evaluation and treatment. Without timely intervention, the appendix can rupture, leading to life-threatening complications.

Typical Symptoms

Symptoms of appendicitis often develop over a period of hours and may rapidly worsen. The most characteristic signs include:

  • Abdominal pain: Initially around the navel, then migrating to the lower right abdomen (the area known as the McBurney point)
  • Fever: Typically low to moderate (37.5 – 38.5 °C), higher if complications develop
  • Nausea and vomiting: Often accompanying the abdominal pain
  • Loss of appetite: A common early warning sign
  • Changes in bowel habits: Diarrhea or constipation may occur
  • Tenderness on pressure: Intense pain when pressing or releasing pressure on the lower right abdomen (rebound tenderness)

Atypical Symptoms

In children, elderly individuals, and pregnant women, the presentation of appendicitis can differ. In pregnant women, the enlarged uterus shifts the appendix upward, so pain may occur in the middle or upper abdomen. In young children, general discomfort, crying, and abdominal rigidity are often the primary signs.

Causes of Appendicitis

The most common cause is a blockage inside the appendix, which can result from:

  • Hardened stool (fecaliths)
  • Enlarged lymph nodes due to infection
  • Foreign bodies or parasitic infections (rare)

The blockage allows bacteria to multiply inside the appendix, leading to inflammation. Without treatment, a perforation (rupture) can occur, which is a life-threatening emergency.

Diagnosis

A doctor will take a detailed medical history and perform a physical examination of the abdomen. Common diagnostic steps include:

  • Blood count: Elevated white blood cells (leukocytes) indicate infection or inflammation
  • Ultrasound: Visualization of an enlarged or inflamed appendix
  • CT scan: Used in unclear cases or when complications are suspected
  • Urine test: To rule out a urinary tract infection as the source of symptoms

Treatment

The standard treatment for appendicitis is surgical removal of the appendix, known as an appendectomy. This can be performed as a minimally invasive procedure (laparoscopy) or as open surgery. In mild cases, antibiotic therapy may be considered, but surgery remains the safest approach to prevent recurrence and serious complications.

When to Seek Immediate Medical Help

The following warning signs require immediate emergency medical attention:

  • Sudden, severe abdominal pain that rapidly worsens
  • A rigid, board-like abdomen
  • High fever above 39 °C
  • Loss of consciousness or circulatory collapse
