What Is the Approximal Surface?

In dentistry, the approximal surface (also called the proximal surface) refers to the side of a tooth that faces a neighboring tooth. Each tooth typically has two approximal surfaces: a mesial surface (facing toward the center of the dental arch) and a distal surface (facing away from the center). The narrow gap between two adjacent teeth is known as the interproximal space or interdental space.

Anatomical Significance

Adjacent approximal surfaces are in close contact with one another at the contact point. This contact point is clinically important because it limits food accumulation between teeth and contributes to the overall stability of the dental arch. When a tooth is lost, neighboring teeth may drift into the resulting space, potentially altering the bite.

Approximal Caries

The approximal surfaces are among the most cavity-prone areas of the teeth, as they are difficult to reach with a regular toothbrush. Dental plaque and food debris tend to accumulate there, where bacteria convert them into acids. These acids attack the tooth enamel, leading to approximal caries (interproximal cavities), which are often detected late because of their hidden location.

Periodontal Health

The approximal area also plays a significant role in periodontitis (inflammation of the tooth-supporting structures). Gum inflammation frequently starts in the interdental region, as proper cleaning is most challenging there and bacterial biofilm can accumulate easily.

Cleaning the Approximal Surfaces

Because a standard toothbrush typically cannot adequately reach the approximal surfaces, dental professionals recommend supplementary cleaning aids:

  • Dental floss: The classic tool for cleaning between teeth
  • Interdental brushes: Small cylindrical or tapered brushes, especially effective for wider interdental spaces
  • Water flossers (oral irrigators): Remove food particles using a pressurized water stream
  • Interdental sticks / toothpicks: Used for targeted cleaning and gentle gum massage

Regular cleaning of the approximal surfaces is an essential part of daily oral hygiene and can significantly reduce the risk of cavities and gum disease.

Diagnosing Approximal Caries

Approximal caries is often difficult to detect visually, as the affected surface is not directly accessible to the naked eye. The following diagnostic methods are commonly used:

  • Bitewing radiograph: The standard imaging technique for detecting approximal caries in the posterior teeth
  • Transillumination (FOTI / DIAGNOcam): A light-based method that can reveal carious lesions without radiation
  • Visual and tactile examination: A supplementary method performed by the dentist

Treatment of Approximal Caries

Treatment depends on the extent of the carious lesion. Early-stage lesions can often be managed non-invasively through professional fluoride application and improved oral hygiene. More advanced lesions require restorative treatment, such as a composite filling or an inlay.
