What is Aromatase?
Aromatase (also known as CYP19A1) is an enzyme belonging to the cytochrome P450 family. It catalyzes the conversion of androgens – male sex hormones such as testosterone and androstenedione – into estrogens such as estradiol and estrone. This biochemical process is called aromatization. Aromatase is present in both the female and male body and fulfills important physiological functions in both sexes.
Location in the Body
The aromatase enzyme is found in various tissues throughout the human body:
- Ovaries: The primary site of estrogen production in premenopausal women
- Adipose tissue (fat cells): A major source of estrogen production after menopause
- Liver: Involved in hormone metabolism and regulation
- Brain: Local estrogen production influences neurological functions
- Testes: Regulates estrogen levels in men
- Bone and muscle tissue: Influences bone density and muscle metabolism
Mechanism of Action
Aromatase catalyzes a biochemical reaction in which the A-ring of the steroid backbone is aromatized. Specifically:
- Testosterone is converted to estradiol (E2)
- Androstenedione is converted to estrone (E1)
This process requires oxygen and NADPH as cofactors and proceeds through three hydroxylation steps. The encoding gene CYP19A1 on chromosome 15 controls the synthesis and activity of the enzyme.
Medical Significance
Aromatase in Women
In premenopausal women, estrogen is primarily produced in the ovaries through aromatase activity. After menopause, adipose tissue takes over this function. Elevated aromatase levels can lead to an excess of estrogens, which is associated with certain medical conditions.
Aromatase in Men
Men also require small amounts of estrogens for bone metabolism, cardiovascular health, and libido. However, excessive aromatase activity in men can lead to elevated estrogen levels and may cause gynecomastia (enlargement of breast tissue in men), among other effects.
Clinical Relevance and Associated Conditions
Aromatase Deficiency
A congenital aromatase deficiency (caused by mutations in the CYP19A1 gene) results in the absence of estrogen production. In women, this manifests as absent pubertal development, primary amenorrhea (absence of menstruation), and infertility. In men, it leads to accelerated bone growth without premature closure of growth plates.
Aromatase Overactivity
Increased aromatase activity is associated with the following conditions:
- Hormone-dependent breast cancer: Many breast cancer types grow under the influence of estrogens produced locally by aromatase
- Endometriosis: Elevated local estrogen production due to aromatase overactivity in endometrial tissue
- Gynecomastia in men
- Obesity: Increased adipose tissue raises systemic aromatase activity
Aromatase Inhibitors
Aromatase inhibitors are medications that block aromatase activity and thereby reduce estrogen production. They are primarily used in the treatment of hormone receptor-positive breast cancer in postmenopausal women. The most well-known aromatase inhibitors include:
- Anastrozole (Arimidex)
- Letrozole (Femara)
- Exemestane (Aromasin)
These agents reduce estrogen synthesis by up to 99% and are an important component of adjuvant hormone therapy in breast cancer treatment.
